Casualty report states: unable to obtain power - AWS change end switch handle in wrong position

Driver Coultish (York) reports: On taking charge of loco 9020, which was already attached to the
train, the King's Cross relief driver informed me that the loco was all in order, but the brake test
wanted carrying out
. After completion of the brake test and on getting the signal to start I opened
the controller but no amps were available
. I quickly saw that the AWS change end switch was in the
down position, but on cancelling the horn the brake did not release
. I therefore proceeded to the
opposite cab and found that the AWS change end switch had been left in the up position, presumably
by the King's Cross driver. After putting the switch in down position and returning to my driving cab
everything was in order
. The total time for this operation only took two to three minutes, but the
signal had by that time returned to red, in order to let another train into the station, so I had to wait
another five minutes or so before the signal cleared
.

working correctly from King's Cross to Corby the ETH failed cutting off power to the locomotive

ETH restarted but same thing happened so wired for fresh loco from Grantham
.
Conclusions: 28.01.72 DME X states: I can only assume that there was a fault on the coaching stock
(Mark IID) causing the power earth fault relay to pick up
. This casualty should therefore be cancelled
.

Conclusions: 11.04.72 DME KX states: This locomotive was subsequently checked in service by the
CM&EE's representative Mr Edwards but could not be faulted
. The trouble as experienced would
suggest defects in the auxiliary generator output/battery charging circuits
. In this connection I would
draw your attention to subsequent casualty to this locomotive on 21st March when No.1 auxiliary
generator drive was found to be defective, although any connection between the two incidents is
doubtful
.

contacts, also loop wire E7 broken at crimped connection (locomotive subsequently hauled train to
Welwyn Garden City on No.2 engine only)

Driver L Booth (King's Cross) reports: Engine low speed, driver operated through broken wire,
regain power on one engine after having shut down defective engine, ready to proceed in 5 minutes,
however excessive time taken to regain staff back onto train and loco change at Welwyn, incidentally
had not requested fresh loco until Hitchin or Peterborough, whichever place was suitable for Control
.

Conclusions: DME KX states: This locomotive had been released from Doncaster Works Classified
Repair on 23rd February, where presumably the incorrect wiring had been made. I enclose to the
CM&EE the broken loop wire, no doubt he will take up with BREL

I can only assume that there must have been a degree of mismanagement, possibly due to the
unfamiliarity with the vigilance system and that the ETH defect must have been in the coaching stock

was cancelled satisfactorily, the ETH was also tested satisfactorily from each end, loco could not be faulted

casing & battery charge contactor tips splashed at one side thus not allowing contactor tips to
close fully

Conclusions 09.05.72 DME KX states: Since the frequency of the examination of battery charge
contactors was increased to 'C' examinations there has been very little trouble experienced
. I do
not therefore consider that any further action is necessary regarding this item
. I am unable to
comment further regarding the broken auxiliary drive belt, as I have no information other than
shown above
. The locomotive had however received a classified repair only a month earlier
