Casualty report states: Driver G. Elliott reports "On 13.01.70 I worked 1L16 down & 1A33 up with 9007, both fuel & water tanks full when leaving King's Cross. At Leeds City 200 gallons boiler water registered on gauge. The driver I relieved on the up train at Leeds City said he noticed water leaking from the tank as it neared top when being filled. By the time we passed through Peterborough the water gauge registered less than 100 gallons. Soon after passing Abbots Ripton a fault light indicated low fuel. I had the boiler shut down and as I passed St. Neots & Sandy signal boxes I blew the horn and signalled that I wanted a fresh engine. When I examined the engine on stopping at Hitchin water was leaking from holes underneath. One fuel tank was on zero, the other showed 100 gallons. At no time on the road did I notice any bump or anything to place where this damage could have happened."

The Araldite joint was remade (previous casualty on 28.12.69 states this joint remade at Gateshead)

Follow up report dated 20.03.70 states: This locomotive had just been released from Finsbury Park depot following repairs from a previous casualty due to lighting fuses blowing. It would appear that insufficient attention was given after the earlier casualty as further wiring in the conduits to the charging and lighting sockets was found to be burnt out after this second casualty. Suitable notice has been taken with staff concerned.

4S42 21:05 Dagenham - Bathgate, (from Finsbury Park to Doncaster) (delayed 3 minutes at New Barnet, 7 at Hatfield & 10 at Hitchin).
Casualty report states: No.1 engine cutting out - header tank low in coolant (header tank topped up)

Driver J. Crate (King's Cross) reports "The first fault occurred when I closed the controller to reduce speed for the 30 mph speed restriction at New Barnet on the down main line. A bright blue light appeared on the drivers desk. This was No.2 engine showing high water temperature.
I could not immediately regain power when I re-opened the controller and brought the train to a stop at New Barnet No.1 home signal. After opening and closing the controller twice I regained power also blue light dimmed. The second fault occurred on the approach to Hatfield, a bright blue light reappeared on the desk due to No.2 engine showing high water temperature again. When closing the controller No.1 engine cut out and stopped for no apparent reason. They advised stopping at Hitchin for a check by fitters. I was able to re-start No.1 engine which gave no further trouble and I opened all engine compartment windows to try to assist cooling of No.2 engine. After a check by fitters at Hitchin the locomotive gave no further trouble apart from No.2 engine continuing to show a bright blue light when working hard. The load of this train was 13 Cartics total tonnage 699."

Driver F. Hudson (King's Cross) reports "Fire caused by electric flashover between Carlton and Cromwell signal boxes, smoke entered cab approaching Newark Crossing distant, so I immediately braked train to about 15mph and asked second man to see the reason. He reported some flames by engine room step, this was dealt with by carbon dioxide bottle and I kept train moving slowly to clear main to slow road at Newark north to enable trains behind mine to have a path. When I stopped at I investigated and saw that smouldering was taking place in an inaccessible place, so fire brigade had to deal with it.

Low fuel light not working - locomotive returned to Finsbury Park where it was found that the fault light relay was defective and had to be renewed.
Recommendations: Even if the fault light relay had not been defective a casualty would not have been avoided. This should
therefore be coded as non-mechanical and no doubt the depot engineer at Haymarket will investigate the apparent lack of servicing.

Casualty report states: fumes in engine room and smoke from lagging around No.2 engine exhaust drum (drum tank lagging removed at Finsbury Park & tested no gas leak found)
. Driver A. Martin (King's Cross) reports "No.2 engine shut down. Smoke emitting from lagging around exhaust drum. Further examination showed nothing smouldering after No.2 engine had been shut down for some time." Conclusions: see casualty for this locomotive two days earlier. This second casualty would have been avoided if the driver of the first casualty had made an entry in the drivers repair book as the locomotive would not then have been allowed into service without being accompanied by technical staff. With regards to the smouldering of the exhaust drum tank lagging, I would point out that this locomotive had only been released from Doncaster Works on 18.09.70
after attention to flashover damage, though there is nothing in the repair report to indicate that the lagging was touched.

31.12.70 Conclusions: Wheel wear compensator was deliberately adjusted to 49" as there was no speedometer available at York and it was found by staff that the speedometer could be made to register correctly by altering the compensator advising
further attention at home depot. This was done and it was found necessary to adjust the compensator back to 43". The speedometer could not be faulted and there were no further reports to the locomotive
