Conclusions: 12.02.76 DME KX states: I am puzzled as to say why a mechanical casualty has been initiated for this incident, particularly as the locomotive worked to King's Cross to complete its diagrammed working. I suggest therefore that the casualty should either be cancelled or if seven minutes delay was incurred before the AWS was isolated then this should be taken up with the driver & the casualty coded as Non-mechanical. Conclusions: 12.02.76 DME KX states: Ill fitting cab doors and cab doors which will not stay closed are a continuous source of trouble on this class of locomotive, but unfortunately due to the good will of drivers they do not usually result in casualties. The problem seems to stem from a combination of the locks, catches and draught proofing. I can only again request special attention to all details at main works overhaul. Finsbury Park TMD. Conclusions: 08.09.76 DME KX states: The obvious conclusions from this casualty is that the locomotive had not been properly serviced at King's Cross prior to working this train & this is being taken up with the staff responsible. However in view of this report and the previous casualty on 15th May the boiler water tanks were specially checked during examination at Finsbury Park on 29th May & although the suction strainer joints were renewed no real defects were found. It is noted however that when the locomotive was sent to Doncaster Works for Unclassified Repair on 03.06.76 two leaking water tanks were found. |

It was noted that there was considerable corrosion inside the junction box which was bad enough to make it necessary to renew the terminal post for the train heat control loop wire connections. However in view of the fact the brake had apparently functioned correctly since that time I find it difficult to hold the fitter concerned at fault. |

Conclusions: 29.09.76 DME KX states: Since this incident the locomotive has been checked in traffic by a Technical Inspector who reported it was somewhat rougher in riding than average and also specially examined at Finsbury Park but no apparent cause found. It appears to be worse at No.2 end and a replacement set of dampers has therefore been obtained for fitting to No.2 bogie at the next examination, or if there are any more bookings of rough riding. |

It has not however been possible to ascertain any reason why this particular locomotive has been in trouble, but this tendency has also been noted on other locos of this class. Perhaps this is a matter worthy of further general investigation. Subsequent examination at Finsbury Park on 30.10.76 revealed accumulation of water in No.1 end ETH jumper cable connection box. At the 'B' examination on 8th December the main generator insulation readings were found to be low. Subsequently the locomotive had to be shopped again on 16th December due to engine aerating and the opportunity was taken to change both main generators. Gateshead TMD. Repairs - flashover damage cleaned up, insulators and flashguards renewed as necessary, security of connections checked & traction motors examined (not determined if carried out at GD or FP). |
