Conclusions: 25.03.74 DME KX states: No repairs were carried out at the time but burner motor subsequently changed at Finsbury Park and defective machine forwarded to Doncaster Works for inspection under the 'Red Label' procedure. I am treating this casualty as Non-mechanical. No doubt the DME Leeds will follow up with his Control Organisation regarding misuse of the locomotive. |

After three attempts to start and King's Cross Control being advised, the train was worked by 55012 on instructions of Control (no ETH loco was available at KX). I was told an ETH loco would be provided en-route but it was not - 55012 worked train to York and through to Newcastle. 87,000 miles since last classified repair (13.09.74). Gateshead TMD. |

Conclusions: 16.05.74 DME KX states: The radiators to these locomotives are in run down condition and any slight internal blockage can result in the engine overheating. It is considered that a campaign change of all radiators should be made at an early date before the advent of the hot weather. Doncaster TMD. To Doncaster Works - Unclassified repair. No.2 power unit 422 piston through crankcase - 422 replaced by 457. The E.T.H. and temperature probe modifications were carried out and radiators and thermostat by pass valves replaced. |

Conclusions: 14.01.75 DME KX states: The previous inspection of these contacts would have been carried out during Classified Repair. A check on the locomotives repair history reveals that on 29th November it was necessary to renew the No.2 load regulator control valve 'O' rings due to bad lubricating oil leakage and I can only assume that the lubricating oil may have affected the contacts. King's Cross Loco. Repairs - new load regulator contacts fitted as emergency to return loco to traffic on account of extreme shortage of motive power. |

Conclusions: 14.01.75 DME KX states: Please see previous casualty two days earlier. On that occasion the replacement contacts had been fitted at King's Cross Passenger Loco as due to shortage of locomotives and enginemen, it was not possible to arrange for the locomotive to go to Finsbury Park Depot without incurring a delay in traffic. This meant that the contacts were fitted without the aid of the setting gauge. I am therefore unable to hold any staff at fault. (0B02 Peterborough - Finsbury Park). Finsbury Park TMD. Repairs - Load regulator contacts renewed and set up correctly. |
