Casualty report states: Loss of air pressure - fractured air pipe in No.2 nose end (pipe between brake application unit, DSD magnet valve and relay air valve. Conclusions: 05.09.75 DME KX states: This locomotive was examined by CM&EE representative Mr. Ogden, on 11th August, who arranged for further investigation by design staff. I would draw attention to two other recent casualties of this locomotive due to broken air pipes. The first of these, 17th July, presumably has no relation with this present casualty as the fracture was in an entirely different part of the locomotive. However, the other on 20th August, although it involved a different pipe, was also one of those in the vicinity of and connected to the brake application unit. In view of this the security of the brake application unit has been specially checked but could not be faulted. The only other item of which I am aware that could possibly be relevant is that No.2 traction motor blower was found to be vibrating excessively and this was changed at the same time attention was given to this casualty (No.1 was exchanged at Doncaster 08.07.75). Peterborough TMD. Repairs - temporary repair to air pipe (for further attention at home depot). (0B02 Peterborough - Finsbury Park). Finsbury Park TMD. Repairs - new air pipe fitted. |

Cross (5minute delay Grantham). Conclusion: 15.10.75 DME KX states: I can trace no record of any information being passed to this division at the time of this incident & therefore there was no follow up to specially investigate. Also there is no record on the locomotive's examination cards of this trouble. I am unable, therefore, to draw any useful conclusions, but I would point out that this particular locomotive was fitted with new radiators in April last (at 'D' exam). Holbeck TMD. 'A' exam. 1Axx Leeds - King's Cross. 1S35 14:00 King's Cross - Aberdeen (26minute delay King's Cross - 55008 forward). Casualty report states: Driver reported o.1 end driver's side cab door not closing properly (5 feet of door sealing strip missing). Conclusions: 02.01.76 DME KX states: A new section of sealing strip was fitted and further attention given at Finsbury Park on 08.12.75 to try to obtain satisfactory seal but following further complaints the locomotive had eventually to be returned to Works for rectification on 12.12.75. Please note that this locomotive was released from Classified Repair on 02.12.75 and the door had been previously booked by the CM&EE as defective on trial run. This is all too frequent trouble with this class of locomotive and only highest standards of attention at Main Works repairs would seem satisfactory to prevent troubles in service. 1,600 miles since last classified repair (01.12.75).
