Conclusions: 19.01.76 DME KX states: My control records show that the driver requested a new lamp in the instrument panel to illuminate the speedometer gauge before he left Peterborough. This request was passed to Regional Control and it is a pity therefore that a replacement bulb could not have been fitted in traffic and a casualty avoided. Doncaster TMD. Repairs - panel light bulb renewed.

5 -	Finsbury Park TMD. Locomotive taken out of traffic - awaiting works attention. No shopping proposal to hand but it is to be assumed 55012 failed with 1E07 on 03/01 as No.2 engine was pegged out. Loco was oaky to works on No.1 engine only. 3L21 22:15 King's Cross Goods - Leeds (51 minute delay King's Cross Goods - 47255 forward). Casualty report states: Driver reported locomotive low in coolant, blue alarm light indication received - at this time the locomotive was en-route to Doncaster Works with No.2 engine pegged out, for a power unit change, and it was this engine which was in fact low in coolant - No.1 engine was satisfactory. Conclusion: 24.02.76 DME KX states: This casualty was a misunderstanding of the operation of the fault alarm system by the driver. Perhaps consideration should be given to either an instruction to enginemen regarding the need to ignore the general alarm light indication when one engine is 'pegged out' due to low coolant, or alternatively depots should be instructed to wedge open the relevant warning relay contacts.

Conclusions: 07.05.76 DME KX states: It is known that the reliability of the windscreen wipers on this class of locomotive is not very satisfactory and I understand that modification No.MB211/163 has been proposed to fit a simpler arrangement. Gateshead TMD. Repairs - wiper blades set to correct position and tested satisfactorily.

Conclusions: 25.05.76 DME KX states: Unable to fault on arrival at King's Cross or when examined at Finsbury Park Depot. The driver's repair booking indicated that the earth fault button had been operated to shut down the engine down. The locomotive was subsequently checked in traffic but could not be faulted and there has been no repeat of this trouble to date. Finsbury Park TMD. Repairs - unable to fault.

Casualty report states: ETH fault - complete loss of power (power regained after switching off ETH). 87,710 miles since last classified repair (01.12.75). Conclusions: 30.06.76 DME KX states: A check on this locomotive's repair book reveals no further reports of similar trouble. The driver's report might very well suggest an earth fault in the ETH circuit of the coaching stock. Gateshead TMD. Repairs - appropriate circuits checked & engine run up and tested but unable to fault.

Letter from DME Doncaster to DME KX dated 13.05.76 states: King's Cross Driver reported, after relief at Doncaster, that a joint had broken on No.1 engine and lubricating oil was leaking. Regional Control was requested to have the locomotive examined at York, who later advised us that a crankcase joint was leaking and the locomotive was now on one engine - no delay was incurred at Doncaster.

Haymarket TMD. 1E26 17:00 Edinburgh - King's Cross, to Newcastle (31 minute delay Alnmouth, 4 minutes at Widdrington and 10 minutes at Newcastle - 47459 forward). Casualty report states: both engines cutting out due to CCB tripping. 105,490 miles since last classified repair (01.12.75). Conclusions: 02.07.76 DME KX states: On return to this Division the locomotive was accompanied in service by a Finsbury Park supervisor but could not be faulted. There have been further similar reports, on 10th June, but again this was intermittent and could not be traced. This is the most difficult type of defect to trace and I am satisfied that all that can be expected, including meeting the locomotive on every arrival at King's Cross has been done. Gateshead TMD. Repairs - circuits tested, coolant lubrication system checked, locomotive given light engine test run but unable to fault.

Casualty report states: persistent high water temperature - it is known this locomotive had second-hand radiators fitted and was running under extreme ambient temperature conditions. Conclusions: 21.09.76 DME KX states: This locomotive was fitted with new coolant radiators by Finsbury Park Depot in April 1975 but unfortunately these were changed for second-hand ones when the locomotive was in works for Classified Repair last December. No.2 engine radiators were exchanged at Finsbury Park on 04.07.76 and the old ones forwarded to works under 'Red Label' procedure. (0B02 Peterborough - Finsbury Park). Finsbury Park TMD. Repairs - both engine coolant thermostatic by-pass valves placed in emergency position. Locomotive restricted to King's Cross-Leeds-King's Cross services until new radiators available.

Mileage was recorded as 91,570 but it was 105,490 on 28.05.76. Conclusions: 17.09.76 DME KX states No.2 engine coolant radiators changed 4th-9th July - replacements on order for No.1 engine & thermostat by-pass valve placed to emergency pending possible change of No.1 radiators. I understand that at the time of casualty it was raining and a gale was blowing and at this point the train is on a rising gradient starting from Berwick. I therefore consider that this was most probably a case of genuine wheelslip and am treating the casualty as non-mechanical. Haymarket TMD. Repairs - main generators and traction motors checked (no fault found). Locomotive put on test run - still no fault found (returned to traffic).

Conclusions: 21.01.77 DME KX states: After bleeding the water pump no further trouble was experienced. This was the locomotive's first working after 'B' examination at Finsbury Park Depot and the staff responsible have therefore been suitably spoken too.

Experiment DL/543 "Electrically operated windscreen washer pump" was fitted. The body was given an Intermediate repair and the route indicator panels were plated over.
