Casualty report states: Driver reported locomotive low in coolant, blue alarm light indication received - at this time the locomotive was en-route to Doncaster Works with No.2 engine pegged out, for a power unit change, and it was this engine which was in fact low in coolant - No.1 engine was satisfactory

Conclusion: 24.02.76 DME KX states: This casualty was a misunderstanding of the operation of the fault alarm system by the driver. Perhaps consideration should be given to either an instruction to enginemen regarding the need to ignore the general alarm light indication when one engine is 'pegged out' due to low coolant, or alternatively depots should be instructed to wedge open the relevant warning relay contacts
.

There have been further similar reports, on 10th June, but again this was intermittent and could not be traced. This is the most difficult type of defect to trace and I am satisfied that all that can be expected, including meeting the locomotive on every arrival at King's Cross has been done

No.2 engine radiators were exchanged at Finsbury Park on 04.07.76 and the old ones forwarded to works under 'Red Label' procedure

Conclusions: 21.01.77 DME KX states: After bleeding the water pump no further trouble was experienced. This was the locomotive's first working after 'B' examination at Finsbury Park Depot and the staff responsible have therefore been suitably spoken too
