Conclusions: DME KX states: I have checked the history of this locomotive and find that although
an 'A' examination was not given at King's Cross before working this train there was attention given
to the brake equipment in that the exhausters had been booked as not going into release speed and it
was necessary to adjust the air pressure switch to correct this. However it would appear that the
casualty at Doncaster was due to the erratic operation of the AV2 valve, probably due to dirt
contamination
. The last attention given to this would have been at Main Works Classified Repair
.

Conclusions: DME KX states: This is not an uncommon problem on this class of locomotive
but the symptoms are usually detected before a casualty is incurred. I assume that the problem
is partly due to the oily atmosphere in which the equipment works (e.g. from breathing engines)
.
I would suggest that this is a matter which should be kept under observation to ascertain
whether more frequent cleaning of the clutch shoes is justified
.

cooler than it was at the time of the casualty. I am not satisfied with the condition of
the coolant radiators for this engine although it is quite probable that no trouble will
be experienced in the winter months
