Conclusions: 21.02.74 DME KX states: I note the attention given at Gateshead Depot but would draw
attention to the fact that the locomotive became a casualty again three days later due to No.2 engine
overspeed trip operating. It was then found that the engine maximum rpm was too high & I am inclined
to think that this was the cause of both casualties

01.02.74, I am unable to state how the governor came to be out of adjustment, but a check on the
locomotive's repair book reveals an entry on 21.01.74 of a union nut in the regulating air pipe being
found loose. I can only suggest that prior to this someone may have tried to overcome the loss of
regulating air pressure by unauthorised adjustment to the governor - although there is no direct
evidence to support this suggestion

Conclusions: 05.04.74 DME KX states: This locomotive subsequently gave further trouble with
very occasional reports of No.2 engine shutting down. When checked in traffic with an ambient
temperature of 50°F-60°F it was found that the water temperature for this engine was rising to
over 190°F. The radiators were removed from the engine and cleaned externally by high water
pressure jet and flushed through internally with a hose at Depot, but this effected little improvement
.
The radiators are now (04.04.74) being replaced by a serviced set obtained from Doncaster Works
.
The radiators removed are being forwarded for inspection under 'Red Label' procedure
.

locomotives are still troublesome components from time to time. Wire fractures would appear to
be caused by the conditions under which they operate, that is, vibration resulting from them being
mounted on the bogies and copper corrosion due to ingress of rain water. I would suggest that the
only satisfactory long term solution is to reposition them inside the locomotive body
.

do occur from time to time. I have previously suggested on many occasions that they should
be recited within the locomotive to remove them from the vibration and possible ingress of
water to which they are subjected in their present position on the bogie. I also understand
that a different design of wheel wear compensator was introduced as a replacement some
two years ago, but presumably unless the DME Doncaster had forwarded the defective
component for inspection under 'Red Label' procedure it will not be known which type this was

Conclusions: 10.10.74 DME KX states: The defective component has been forwarded for inspection
under 'Red Label' procedure on advice note No.DR201, and sent in stores van No.DB756631, despatched
on 16.03.74. In view of the fact that speedometer defects are still an all too frequent cause of casualties
on this class of locomotive I await the Inspector's report with interest. I would also draw attention to the
fact that this locomotive had been released from Classified Repair only three days prior to the casualty
.
