Conclusions: 06.03.75 DME KX states: Please see correspondence under the CM&EE reference
regarding this potentially very serious casualty. It would seem that these spur pins had not been
correctly fitted when the locomotive was re-blocked at Haymarket Depot the previous day. It is
also noted that the locomotive received an 'LD125' examination at King's Cross immediately prior
to working this train. The fitter concerned has therefore been suitably spoken to and the incident
has been drawn to the attention of the other staff at King's Cross

Conclusions: 30.06.75 DME KX states: The DME Newcastle does not comment on whether both or only
one engine was involved. A check on this locomotives history shows that it had several reports of high
water temperature trouble happening in the middle of May. On the 23.05.75 it was shopped for, amongst
other defects, all radiators to change. In the first instance No.1 radiators were changed, but with the higher
ambient temperatures in June there were several reports of high water temperatures on No.2 engine and
No.2 end radiators were changed at Finsbury Park on 12.06.75

Works for further investigation. I understand that the works again secured the fan drive but did not
find any cause for the failures and the locomotive is therefore at present being kept under special
observation for any sign of further loose drive bolts

The Deltic fleet was currently under review, Heavy General Repairs (HGR) were in progress and five
examples (55002, 55013, 55016, 55019 and 55021) would be shopped for this classified repair. It was,
however, found that the HGR program would be too costly for a fleet of locomotives with a short expected
lifespan and after the overhaul of the first candidate, 55016, each subsequent locomotive had its HGR
shortened to include less work. As a result an evaluation process was instigated to determine the length
between classified repairs to the class and 55015 was chosen to run without classified repair for a number
of years. 55015's next classified repair was 29.01.79 when it was shopped for Intermediate overhaul and a
report on the condition of the locomotive can be found with detailed report of work carried out
.

Casualty report states: No.1 engine failed at Grantham, changed blower fuse, but engine cut out
again - one engine only to King's Cross

Driver AG Smith (Gateshead) reports: Approaching Grantham a blue light, accompanied with a
reduction of power occurred, investigation revealed a blower motor fuse to be the cause, the train
was brought to a stand to enable the ruptured fuse to be changed, thus resulting in the delay of 16
minutes

Conclusions: 31.12.75 DME KX states: It is probable that a blower fault light came up when the
blower motor changed over to series operation and this misled the driver into thinking that a fuse
had failed. We are then left with the problem of finding No.1 engine shut down in the first place

Following this casualty the locomotive was kept under close observation confined to
King's Cross-Leeds-King's Cross workings for a few days but the fault did not recur
.
