Conclusions: 12.02.76 DME KX states: The previous 'A' examination of this locomotive had taken place at King's Cross Depot, the fitter concerned will therefore be suitably spoken to. |

Note in official records from Driver Coultish reports: Copy Report 18.03.76 - Sir, Re your report 7 minutes delay at Carlton with 1L05 08:10 King's Cross to Leeds, the information you have is all wrong; first the delay was at Cromwell not Carlton and the delay was about 3 minutes, not 7, and I have no knowledge of one engine overspeed trip operating but the temporary lass of power was caused by ATPG. I operated the trigger manually and power restored, this took two minutes. Given the report is dated 18/03 I have credited as 13/03 as 1L05 08:10 ex King's Cross was a Saturday move. |

1N42 23:55 King's Cross - Newcastle, to Peterborough (30 minute delay
Peterborough - 31264 forward 30/03). Casualty report states: boiler defective - low water level, due to boiler water pump air locked (suspect strainer joints). 85,640 miles since last classified repair (21.10.75). Conclusions: 07.05.76 DME KX states: Since the above attention there have been no further reports of boiler trouble to date. Generally speaking the boiler water feed systems on Class 55 locomotives are satisfactory and I do not therefore have any recommendations to put forward. |

Casualty report states: No.2 engine cut out - engine aerating (suspect cracked liner). 99,063 miles since last classified repair (21.10.75) - believed to be mileage at 07.05.76 when TCR's finalised. Conclusions: 07.05.76 DME KX states: Please see also previous casualties, it is unfortunate that satisfactory action was not taken after the first or at least the second casualty to investigate further the cause of coolant loss, but it is significant that these occurred over the Easter Holiday period when there were no Maintenance Controllers on duty in Divisional Control & only minimum staff in Regional Control. | 6 - | 1E26 17:00 Edinburgh - King's Cross,
to Newcastle (8 minute delay Newcastle - 47551 forward). Casualty report states: No ETH indication - ETH indicator light bulb blown. Conclusions: 26.07.76 DME KX states: I cannot accept that this was a valid reason for failing a locomotive en-route as surely a check on the indication in the other cab or observance of whether the motor alternator sets on the coaches were running would have confirmed if all was well. Gateshead TMD. Repairs - new ETH indication light bulb fitted, circuits checked. 1A47 22:20 Newcastle - King's Cross, to Doncaster (10 minute delay Doncaster - 47429 forward 07/05). Casualty report states: heavy exhaust and flames from boiler - boiler overheating due to low fire fuel solenoid valve seat defective. Conclusions: 25.05.76 DME KX states: The defective fuel solenoid valve had been found during servicing at King's Cross the previous day and the locomotive had been retained in traffic with the boiler inoperative pending repair at Finsbury Park. This fact had been advised to Regional Control at the time. This is misuse of locomotive as it was in service with the boiler known to be defective. Low fire fuel solenoid valve later changed at Finsbury Park. |

Casualty report states: Amps fluctuating - 4 load regulator contacts badly pitted and burnt between 8 o'clock and 9 o'clock position. Conclusions: 21.09.76 DME KX states: This particular locomotive has a long history of load regulator contact difficulties. Despite considerable investigation including a check on resistance values using a Wheatstone's Bridge supplied by the CM&EE it had not been possible to locate the cause and I can only assume therefore that there is an intermittent defect. |

Conclusions: 22.09.76 DME KX states: I note that this locomotive should have received an 'A' examination at King's Cross but there was insufficient turn round time for this. It is unknown if 55015 worked to Edinburgh after release from Works or picked up 1E39 at Doncaster. |

Conclusions: 01.11.76 DME KX states: Trouble is experienced with these switches sticking open at times but I do not consider that it is necessary to fail a locomotive for such a fault which should have been found as part of the driver's fault finding procedure. The switch was satisfactory when checked at the 'D' examination and do not consider my staff to be at fault. Gateshead TMD. Repairs - AATP renewed. |

Conclusions: 22.11.76 DME KX states: This locomotive was working to Doncaster Works for a power unit change on No.2 engine only and it was No.1 engine which was in fact low in coolant. This revealed that No.1 engine was aerating badly and the locomotive was taken out of traffic for power unit change, I can only conclude therefore, that aeration had commenced three days earlier. |
