Conclusions: 03.04.75 DME KX states: I would draw attention firstly to the fact that this locomotive had been released from Classified Repair less than two days earlier, and secondly the booking on the CM&EE's ex Works Riding Report dated 20th March stating: "Steam leaking from crane coupling". No doubt the CM&EE will take up with BREL. |

Letter dated 25.04.75 from DME Doncaster to DME KX states: Further to my letter of 16th April 1975: Driver Lister has been seen and states he does not know where the impact occurred. He first noticed the fuel leak when he stopped at Yaxley Crossing to collect information concerning the passing of signals associated with the current re-signalling work, although he is sure that the locomotive was in good order at King's Cross. He does not recall running over any obstructions or hearing any loud knocks en-route from London. |

Conclusions: 22.05.75 DME KX states: I note that at the time of this casualty the locomotive had worked the maximum number of working days towards its first 'C' examination after Classified Repair - it is difficult therefore to hold either Main Works or Depot staff at fault. York TMD. Repairs - No.2 exhaust drum joint remade and lagging renewed. |

Casualty report states: boiler defect - low water Mowbray float sticking. Conclusions: 23.10.75 DME KX states: Since the all round improvements made some years ago in the treatment of water supplied on the East Coast Main Line failures of the type above have been most unusual. However, for a few days prior to this casualty locomotives had been receiving untreated water in King's Cross Passenger Loco due to a water main failure & I would suggest that this coupled with slackness in applying the boiler blow down procedure by operating staff had a direct bearing on this incident. |
