Conclusions: 21.11.74 DME KX states: I regret that the components removed were not forwarded
for inspection under 'Red Label' procedure. I would however point out that speedometer equipment
defects are one of the most prolific cause of casualties on class 55 locomotives and I would suggest
that a thorough design investigation would be well worthwhile.

Casualty report states: No.1 engine cut out - low coolant level due to hose No.12 leaking & high
water temperature.

Conclusions: 27.11.74 DME KX states: This locomotive was subsequently checked in traffic with
the thermostat bypass valve reverted to its normal position but it could not be faulted. I can only
assume therefore that the shut down was due to loss of coolant rather than high water temperature
& it would have been interesting to know whether the driver made any attempt to use the emergency
hand pump. I also note that the locomotive received an 'A' examination at Haymarket immediately
prior to working this train. I am therefore sending a copy of this casualty report to the Depot
Manager, Haymarket, who may wish to take up with the staff concerned.

Conclusions: 10.12.74 DME KX states: I understand that Main Works are re-positioning this
particular pipe as locomotives pass through shops, so as to place it in a less vulnerable position - this
locomotive had not yet received that attention.

Casualty report states: No.2 engine shut down - No.2 fan shaft uncoupled due to bolts missing (loco
through to Leeds on one engine only).

Conclusions: 05.12.74 DME KX States: These bolts are checked at '' examinations. At the time of the
casualty the locomotive was due, but not overdue examination. The fitter responsible for the previous
examination has therefore had this casualty brought to his attention but formal disciplinary action is
not considered to be warranted. Temporary bolts fitted at Holbeck Depot were later replaced at
Finsbury Park.

KX-Edinburgh from Doncaster but became a casualty at Newcastle with No.1 engine defective.
Reported as B2 conrod fractured, it would be interesting to know what was found on No.1 engine
strip down, and whether it is thought that there may have been a defect which could have any
bearing on this first casualty.

and boiler 1493/J2949 replaced by 5097/J3248. The body was given an Intermediate repair. It is believed
the domino headcode blinds were also fitted during this works visit.
