Conclusion: This is a defect which does not occur time to time on these locomotives, although as far as I am aware this is the first time a casualty has been incurred. Also I would point out that this locomotive has a particularly bad record over several years of ill-fitting doors and windows and distorted cabs, with a recent incident of a fracture several inches long between the windscreens and the cab side & special attention to the cabs was requested on the shopping proposal for the last classified repair

Follow up report dated 31.05.72: Immediately following this casualty the thermostatic by-pass valve was suspected and changed and the radiator blocks cleared externally at depot. This however did not prove effective and following a repeat of this trouble the locomotive was sent to Doncaster Works for radiator change, since which it has performed satisfactorily. In view of the troubles with radiators on this locomotive and also 9001 a few weeks earlier, perhaps the CM&EE will follow up with BREL regarding the general question of radiator attention in works and defects found on locomotives 9001 & 9007 in particular

Conclusions (22/08): The hose which was leaking was in the radiator roof compartment and one which in practice is not normally noticed during the course of 'A' examination. I would therefore suggest a rewording of the examination schedule to effect that if there is insufficient coolant left in the overflow tank to top up each header tank with the emergency hand pump then a special examination should be carried out for coolant leaks (this would of course include inspection of all hoses).
