Conclusions: The defective component has been forwarded for inspection under the 'red label' procedure. Examination of radiators is specifically called for under the item 1 (g) of the examination schedule and the last such inspection was earlier in the day at Holbeck, after which the locomotive worked Leeds - King's Cross and was  working King's Cross - Edinburgh when it became a failure. There is of course no evidence as to whether the leak was present at this 'A' examination, but the DME Leeds may wish to draw this casualty to the attention of the staff concerned

Conclusions: Due to late running of the up working into King's Cross (12:30 Leeds - KX) there was insufficient time for refuelling. Also no advice has been received through the control organisation that the locomotive had not been refuelled according to diagram in Leeds. Nevertheless I accept that my control should in the circumstances have specially checked that the locomotive had sufficient fuel to reach Leeds, and I have taken up with the staff concerned accordingly.

Conclusions: I am advised by the DME Newcastle that the cable that was off its terminal had an amp tag fitted which had been made into a spade terminal for some reason or other. Please see casualty to locomotive 9018 on 19.09.73 which was also due to defective wiring in an engine underspeed switch. This wiring & connections have always been a potential source of trouble & in view of these two recent casualties I am arranging for a campaign check on all locomotives allocated to this division (DME KX)

Conclusions: Please see my letter dated 9th November 1972 regarding grease found in the overspeed governor of locomotive 9001, and its possible effect on the resetting of the pawl. following this latter incident it is understood that Doncaster Works have been greasing these governors by mistake, and a campaign check is now in progress on Finsbury Park allocated locomotives. During the course of this it was found that No.1 engine overspeed governor of locomotive 9007 was heavily greased. I therefore await 'Red Label' inspection report on the Martinair unit with interest, as if no fault is found I would suggest that greasing of the overspeed governor was the cause of this casualty (DME KX).

Conclusion (03.01.74): I note that there have been at least two instances of main generator armature failures on this class of locomotive recently. The other one of which I am aware involved 55002 on 12th December. No doubt the CM&EE will follow up specially with BREL to establish the cause of these failures (DME KX)
