20.03.74 - Motive power Inspector (Doncaster) states: Driver EH Coultish is a driver transferred to Doncaster from Goole & until recently learning the route to King's Cross had no experience over this section of line, therefore would not be familiar with the watering points located at Peterborough, there is no reference to these points in the Sectional Appendix or SOI although they do exist on the south-east of platform No.2 and north-east of No.3 platform
. According to the Southern Area Sectional Appendix (page 332) the hydrant covers are painted green so that they may be readily distinguished. The covers at Peterborough are not painted and are difficult to distinguish. In my opinion the instructions should be amended in the Southern Area Appendix to include Peterborough and the covers painted green. This would draw attention to drivers that hydrant points still exist.

Conclusions: 21/06 DME KX states: see also casualty for previous day. Following the return of this locomotive to the King's Cross division the No.1 header tank was checked for a hole in the topping up stand pipe but could not be faulted. It was ridden with in traffic on 11th June after further reports of high water temperature on No.1 engine on both 6th & 10th June. It was then found that the engine experienced a high water temperature shut down at 190°F with 18°F differential across the radiators. The locomotive was therefore accepted on 17th June for main works attention to No.1 radiators

09/07 DME Newcastle states: I note the concluding remarks made by DME KX that Gateshead staff are responsible for the failure of locomotive 55007. This was refuted and suggested that staff at Finsbury Park were at fault during its last 'B' exam 28.05.74.

Driver Handles (Gateshead) was interviewed to establish reason for delay
. 55007 is fitted with experimental DSD equipment and had to be isolated because of intermittent trouble before passing Doncaster. The loco was whistled off prior to York in order to give control enough time for a loco change. Driver Handles did not want to hand over a defective loco to the relieving set at Newcastle without an attempt to replace it. It was not his decision for the loco to come off at York, he acted on the instructions received. Driver Handles was reminded that whistling off indicated loco failure and control no doubt acted upon this.
Conclusions: 26/09 DME KX states: I consider that in these circumstances the driver should have isolated the DSD and proceeded forward, so as to avoid delay and a casualty

DSD fault at No.1 end, DSD pedal micro switch defective and No.2 bogie 9000-34 No.5 wheelset with flats and R/H/S tyre holed causing rough riding - power units No.1 410 replaced by 413 & No.2 434 replaced by 457, bogies No.1 9000-33 replaced by 9000-23 & No.2 9000-34 replaced by 9000-30 and boiler 5095/J3246 was given a standard repair. The body was given a Light repair. All defects were rectified. Experimental window wiper motors fitted.

Conclusions 16.01.75 DME KX states: I note the details given by DME Newcastle but I do not consider that 15 gallons of lubricating oil is significant and the cause of this casualty. The locomotive was again in trouble three days later with No.2 engine shutting down when the LPA plug in the circuit to No.2 engine run solenoid was found to be loose. After giving this attention there have been no further reports of engine shut-down and I am inclined therefore to think this was the true cause of this casualty
