Driver HA Hart (King's Cross) report states: Whilst working 1L16 ex King's Cross the ETH indication light was very erratic, also the engine appeared underpowered. The ETH was switched off and steam heat applied. This made no or hardly any difference to the locomotives performance. The amp meter reading was low indicative of defective diverts, maximum attainable speed was 60 mph. Control was notified and a fresh engine supplied at Peterborough. These defects were reported in repair book but on examining the repair book three days later to find cause of failure I noted that my repair report plus all preceding reports had been torn from book. Will you please look into the removal of these pages as they are helpful in diagnosing failures. 320 miles since last classified repair (22.01.71). |

Conclusions 16.06.71: DME KX states: I have checked the subsequent history of this locomotive and find that it was checked on arrival at King's Cross on 19.05.71 but could not be faulted. It then ran in service without any further reports of trouble until 22.05.71 when No.2 engine was reported as shutting down and this traced to an open circuit in No.2 engine run solenoid, which was renewed at Holbeck depot. I can only suggest therefore that this solenoid may have temporarily gone to open circuit four days earlier and was the cause of this casualty. Gateshead TMD. Following further investigation after this the Martinair cylinder was changed and the defective component forwarded for inspection under the 'Red Label' procedure. In view of the amount of trouble experienced with this item I consider that the standard of main works examination and repair must be suspect and I await the inspection report with interest. |

Shopping proposal states: No.1 main generator commutator damaged by object and groove cut in commutator, suspect earth leakage on No.2 main generator or No.s 1, 2 & 3 traction motors. No.1 engine full load revs 1038, idling 490, these to adjust. No.2 engine idling speed only 445 giving only 765 volts on ETH, this to adjust - loco light engine on No.2 engine only. Mileage was stated as 114,500, slightly less than that stated on casualty report. |

Casualty report states: No.2 engine shut down - temporary high water temperature or loss of coolant (cooling system topped up and engine restarted). Driver Hewitt (Holbeck) reports: No.2 engine stopped as we passed Grantham due to high water temperature. Secondman did not know where pump was, so kept going to see if it would cool down whilst running, but this did not happen. Stopped at Corby to use pump myself but still would not start, asked signalman to have fresh loco at Peterborough. Engine started after Corby but had to stop Peterborough due to having fresh loco ready. Told them I did not require it, we had no more trouble and only two minutes late into King's Cross. 122,480 miles since last classified repair (22.01.71). Conclusions: 03.11.71 DME KX states: The delay between Corby & Peterborough would have been avoided if secondman had known how to use the emergency pump, as arrival at King's Cross was only two minutes late this casualty should be cancelled. |

Conclusions: 30.11.71 DME KX states: A check on this locomotives history reveals that there had been previous trouble due to suspected fan clutch slip, and the shoes had to be cleaned and refitted. It would appear therefore that there may have been leakage from the gearbox and that cleaning the clutch shoes was not sufficient to prevent further trouble. York TMD. Repairs-No.2 radiator clutch and housing renewed. |

Conclusions: 30.11.71 DME KX states: After changing the governor the locomotive was checked in traffic on 9th November when it was found that there was no fuel delivery from No.1 engine 'A' bank fuel pumps. The locomotive was therefore sent to works for attention. |
