Conclusions: 31.03.71 DME KX states: This defect does occur occasionally - I would however draw
attention to the fact that in this particular instance the locomotive had been released from Main
Works Classified Repair only three days earlier
.
Note: casualty report states 28.02.70 as failure date but 27.02.70 for previous exam/repair - it does
however state that both carried out prior to failure

Casualty report states: cab lights going out when indicator lights switched on

Conclusions: 21.04.71 DME KX states: At the time of this casualty the locomotive was being
returned to Works for rectification due to persistent false wheelslip indication
. Following the
casualty examination at Finsbury Park Depot revealed that the lighting circuits also required
attention and this was added to the shopping proposal

Casualty report states: defective boiler - 30 amp fuse blown

Driver H Foster (King's Cross) reports that the 16 minute delay was caused by failure of loco on 1L03
07:55 King's Cross - Bradford, his allocated loco for 1L05 was taken for 1L03 and 9018 allocated to
1L05
. However, boiler failed to function and fitters called for attention - fitters failed 9018 and a
third engine was provided in the form of 1541
.
Conclusions: 23.04.71 DME KX states: As there was a spare fuse on the loco it is considered this
should have been fitted and the casualty avoided. The driver states the loco was failed by the fitters
and not himself - Mr Page & Mr Tumbridge to investigate further please

Conclusions: 09.08.71 DME KX states: This locomotive had been released from Doncaster Works
after No.1 power unit change the previous day - a check and its subsequent history shows further
intermittent reports of high water temperature No.1 engine
. It was checked in service and found
that No.1 engine governor was defective giving excess power at lower than designed maximum
engine speed. This was changed 30.07.71 and sent for inspection under the 'Red Label' procedure
.
Since that date there have been no further reports of high water temperature
.

Driver J Cox (King's Cross) reports: The train was running normal until it reached Hatfield where
I had the distant 'ON' for Welwyn GC and Welwyn North. As I was about to start braking the brakes
went on and I stopped just short of Welwyn GC starter
. As I could hear air escaping in the nose I got
out and examined the pipes, thinking that I had struck something or the communication cord had
been pulled
. Everything was alright but getting back into the cab I found that the engine had stopped.
I sent the secondman to the phone and found the CCB had tripped
. Control had arranged for an
assistant engine and it came on the back and we departed at 18:37
. At Langley 5623 came on the
front and rear loco was removed. 5623 & 9018 were removed at Peterborough and 1766 came on
.

Conclusions: 09.08.71 DME KX states: This was the first trip of the locomotive after No.1 engine
governor had been renewed at Finsbury Park Depot - the defective solenoid coil which was part of

locomotive and for this reason I cannot place any definitive blame on the electrician who carried
out the examination two weeks previously
. Special instructions were issued on 22.11.71 for the
load regulator tip settings and clearances to be checked on 'B' examinations and orders have now
been placed by Finsbury Park for the gauges and tools to do this properly
