released to traffic on 16th June but after working from Doncaster to Hull was again reported as
No.2 engine shutting down
. It was returned to Doncaster Depot for investigation where the fault
was found to be a defective water level switch, which was changed

was its first working, therefore I would suggest that due to the bad weather conditions the exhaust
lagging may have been damp and the reported fault may just have been the lagging drying out - please

. In many of these cases there is really no need for the driver to
fail the locomotive, but it is difficult to be critical of anyone being cautious in these circumstances
.

unduly alarmed the driver

Driver Coultish (York) reports: On taking charge of loco 9020, which was already attached to the
train, the King's Cross relief driver informed me that the loco was all in order, but the brake test
wanted carrying out
. After completion of the brake test and on getting the signal to start I opened
the controller but no amps were available
. I quickly saw that the AWS change end switch was in the
down position, but on cancelling the horn the brake did not release
. I therefore proceeded to the
opposite cab and found that the AWS change end switch had been left in the up position, presumably
by the King's Cross driver. After putting the switch in down position and returning to my driving cab
everything was in order
. The total time for this operation only took two to three minutes, but the
signal had by that time returned to red, in order to let another train into the station, so I had to wait
another five minutes or so before the signal cleared

1S77 23:20 King's Cross - Edinburgh, to Peterborough (3 min delay Stevenage & 19 at Peterborough - 9016 fwd 24/01).
Casualty report states: no exhauster sped up No.1 end - exhauster speed up switch out of adjustment

Driver F Hughes (King's Cross) reports: The vacuum and air brakes on 9020 were tested by myself
before leaving Loco yard and they were normal. But after being coupled to the train vacuum brake
would not rise above 15 inches, I asked the C&W examiner to look at the train and also asked for
our own fitter to examine the loco brake again
. The exhauster exciters would not make any difference
to the vacuum and it took 3 to 4 minutes before I got to 20 inches
. The loco fitter said it was the train
brake and that the locomotive was okay to go
. I had to make a brake application approaching
Stevenage and Holme and was brought to a stand both times
. Control were informed of trouble at
Stevenage and I requested another locomotive from Peterborough
. This defect had been booked two
days before and the fitter remarked 'tested and found okay on shed', the exhauster exciter was useless
when coupled to a train
.
Conclusions: DME KX states: This switch had received attention ten days earlier when the locomotive
was a casualty at Newcastle - it would therefore seem that the check nut had not been tightened
correctly after this first incident and no doubt the DME Newcastle will investigate. However, in view
of these two casualties I have instructed my Depot Manager at Finsbury Park to specially check this
switch at the next 'B' examination

. This was intermittently resulting in sufficient loss of air to cause
a brake application & was no doubt the cause of this casualty

traffic, by Technical Staff on several occasions recently to try to ensure that all defects have

. Even after the attention at Finsbury Park it has again been reported as not
performing completely satisfactorily and the type of fuel solenoid valve fitted, labelled as
ACO/TN/O, is now suspect. This has been raised with the CM&EE's representatives Mr Starkey

were converted to electric train heating

Newcastle - Edinburgh, Depart Newcastle 17:33, leaving 13 late on this occasion. On approaching
Berwick (booked stop) smoke was seen to come from the engine room door, no fire alarm bell had
sounded. After stopping in platform driver opened engine room door to find dense smoke and flames
.
Action taken by driver and secondman, after stopping both engines, was to use fire extinguishers
from opposing cabs. After fire was extinguished it was seen that the location of fire was No.2 exhaust
drum, causing damage to flexible water pipes. After informing Divisional Control of damage it was
decided to terminate train as the 1S35 14:00 KX-Edinburgh was now awaiting a platform
. No.1
engine was restarted and train moved to Berwick yard. Heavy water leak noted from header tank so
it was decided to have loco and train hauled to Edinburgh by ScR loco and crew leaving Gateshead
crew to relieve there intended return working at Berwick

Conclusions: 16.11.72 DME KX states: I note that this locomotive had only just been released after
'C' examination at Finsbury Park Depot. The supervisor responsible for the post examination
inspection has been taken up with and instructed to be more careful in his inspections

leaving Finsbury Park. The fact that the DME Newcastle states the cell was out of position and broken
suggests maltreatment, probably by the footplate staff and I am therefore coding this casualty as

I had the distant 'ON' for Welwyn GC and Welwyn North. As I was about to start braking the brakes
went on and I stopped just short of Welwyn GC starter

out and examined the pipes, thinking that I had struck something or the communication cord had
been pulled
. Everything was alright but getting back into the cab I found that the engine had stopped.
I sent the secondman to the phone and found the CCB had tripped
. Control had arranged for an
assistant engine and it came on the back and we departed at 18:37
. At Langley 5623 came on the
front and rear loco was removed. 5623 & 9018 were removed at Peterborough and 1766 came on

locomotive and for this reason I cannot place any definitive blame on the electrician who carried
out the examination two weeks previously
. Special instructions were issued on 22.11.71 for the
load regulator tip settings and clearances to be checked on 'B' examinations and orders have now
been placed by Finsbury Park for the gauges and tools to do this properly
