Driver F Hughes (King's Cross) reports: After departing from Doncaster, at Decoy, there was six
or seven bangs which I thought were flashovers. They all came within seconds - I closed the
controller immediately and coasted until I stopped at Rossington. I examined No.2 engine and
found that the main cables from the generator had fused in the junction box, which was badly
damaged. I closed down the No.2 engine and requested another ETH locomotive at first available
point, which was Peterborough

Conclusions: 31.10.72 DME KX states: I would draw attention to the increased incidence of failures
of generator field contactors in recent months. Other casualties have involved locomotive No.9007
on 17th June, No.9009 on 6th October and No.9020 on 26th June & 29th July
. It occurs to me that
this may be connected with more arduous use to which they have been put since these locomotives
were converted to electric train heating
. I am therefore instructing my Finsbury Park Depot to
increase the frequency of examination of these particular contactors from 'D' to 'C' examinations
.
Perhaps the CM&EE will also investigate the design aspect
.

Conclusions: 24.10.72 DME KX states: The DME Newcastle advises me that the driver reports that
the train brake released but was unable to move forward. 5856 which hauled the train from the
carriage sidings was brake tested satisfactorily
. Driver Hewson (Gateshead) stated later that after
he coupled 9018 to the train the brake test was in order but when he applied power the train would
not move
. Train finally moved away before attention could be given.

Casualty report states: dense fumes and fire in engine room in vicinity of No.2 engine exhaust
drum - No.2 exhaust collector drum large joint blown out due to stripped studs

Traction Inspector's report states: The above train is worked by Gateshead driver J Lough
Newcastle - Edinburgh, Depart Newcastle 17:33, leaving 13 late on this occasion. On approaching
Berwick (booked stop) smoke was seen to come from the engine room door, no fire alarm bell had
sounded. After stopping in platform driver opened engine room door to find dense smoke and flames
.
Action taken by driver and secondman, after stopping both engines, was to use fire extinguishers
from opposing cabs. After fire was extinguished it was seen that the location of fire was No.2 exhaust
drum, causing damage to flexible water pipes. After informing Divisional Control of damage it was
decided to terminate train as the 1S35 14:00 KX-Edinburgh was now awaiting a platform
. No.1
engine was restarted and train moved to Berwick yard. Heavy water leak noted from header tank so
it was decided to have loco and train hauled to Edinburgh by ScR loco and crew leaving Gateshead
crew to relieve there intended return working at Berwick
.
Conclusions: 16.11.72 DME KX states: I note that this locomotive had only just been released after
'C' examination at Finsbury Park Depot. The supervisor responsible for the post examination
inspection has been taken up with and instructed to be more careful in his inspections
.

casualty the locomotive had to be taken out of traffic for attention to a fractured liner and I am
inclined to think that this was the cause of the first casualty

an 'A' examination at Haymarket Depot

received are concerned, Regional Control were advised by my Maintenance Controller at 02:45 and
a further discussion took place wherein my staff were told that the locomotive should work through
as it was required for further ETH workings
. Some attention was given by my staff to the boiler on
the locomotive but insufficient time was available for further examination
.

that there was a further report of the boiler water pump being defective two days later

investigation then revealed that the connection tag to one of the water pump motor brush arms
was loose
. In so far as staff in this Division are concerned I consider that either defect should have
been noticed and given attention at the 'C' examination on 5th November and suitable notice is
therefore being taken with the electrician concerned
.

Conclusions: 21.11.74 DME KX states: I regret that the components removed were not forwarded
for inspection under 'Red Label' procedure
. I would however point out that speedometer equipment
defects are one of the most prolific cause of casualties on class 55 locomotives and I would suggest
that a thorough design investigation would be well worthwhile
.

Conclusions: 27.11.74 DME KX states: This locomotive was subsequently checked in traffic with
the thermostat bypass valve reverted to its normal position but it could not be faulted
. I can only
assume therefore that the shut down was due to loss of coolant rather than high water temperature
& it would have been interesting to know whether the driver made any attempt to use the emergency
hand pump
. I also note that the locomotive received an 'A' examination at Haymarket immediately
prior to working this train
. I am therefore sending a copy of this casualty report to the Depot
Manager, Haymarket, who may wish to take up with the staff concerned
.

strip down, and whether it is thought that there may have been a defect which could have any
bearing on this first casualty

leaving Finsbury Park. The fact that the DME Newcastle states the cell was out of position and broken
suggests maltreatment, probably by the footplate staff and I am therefore coding this casualty as

went on and I stopped just short of Welwyn GC starter

out and examined the pipes, thinking that I had struck something or the communication cord had
been pulled
. Everything was alright but getting back into the cab I found that the engine had stopped.
I sent the secondman to the phone and found the CCB had tripped
. Control had arranged for an
assistant engine and it came on the back and we departed at 18:37
. At Langley 5623 came on the
front and rear loco was removed. 5623 & 9018 were removed at Peterborough and 1766 came on

Conclusions: 22.12.71 DME KX states: This is an all too frequent trouble with this class of
locomotive and for this reason I cannot place any definitive blame on the electrician who carried
out the examination two weeks previously
. Special instructions were issued on 22.11.71 for the
load regulator tip settings and clearances to be checked on 'B' examinations and orders have now
been placed by Finsbury Park for the gauges and tools to do this properly
