Conclusions 25.08.77 DME KX states: I can only assume that the speedometer generator had been struck in traffic and I am therefore treating this casualty as 'non-mechanical'.

1N42 23:55 King's Cross - Newcastle (69 minute delay Retford & 4 minute delay Doncaster - assisted to York).
Casualty report states: Total failure, compressor fault, loss of main air - suspect compressor governor frozen. Driver reported fault corrected itself after opening nose end door to allow in warm air. Locomotive worked forward from York unassisted. No mentholated spirits in compressor anti-freeze dispenser
.
A note which accompanies this particular casualty report states: BRB instructions are that mentholated spirits should not be used in compressed air systems but local instructions are in force in this division that it should be used in severe conditions, as were prevailing on this particular night. It is interesting to note therefore that this particular locomotive had come off Finsbury Park Depot who had, due to an oversight omitted to fill the anti freeze dispenser, whilst other class 55 locomotives which had been serviced at King's Cross had had their dispensers filled and had worked without trouble. The handwritten remarks at the foot of this note merely state 'Balls'.

This is an odd casualty report (concluded 29.06.78) as the TRN stated is 1A01, though the working is stated as 10:45 KX-Newcastle, it dies say that the fuel pump was renewed, but not where . Also it would appear that this move was in order to get 55007 to works
. Conclusions by DME KX on 28.06.78 are also vague as they state: I note that this locomotive has subsequently experienced a fractured fuel tank (16.05.78), works repairs to same, and fuel strainer/filter contamination problems, but I am unable to associate this with the above casualty.

Conclusions 25.06.75 DME KX states: This locomotive had been released from Finsbury Park after a 'C' examination at 19:00 on 7th June and had stood spare until it worked this train. It is therefore possible that coolant loss from hoses had taken place during this period particularly as these locomotives are not treated with Boscoprene. I would draw attention to the fact that this locomotive had a history of high water temperature troubles until stopped for radiator change on 13th June

This was not lifted until 19th August when called for classified repair - any forays out of the Leeds division were not sanctioned by depot - see following move & casualty on 25/07

Conclusions: 18.09.75 DME KX states: A check on this locomotives records reveals no further failure of this type. I therefore wonder if the engine underspeed switch may have operated & confused the driver into thinking that the engine had shut down

Conclusions: 10.09.75 DME KX states: This would appear to be misuse of the locomotive by control as the boiler water tank was found to be holed during the 'B' examination carried out on 10th July - consequently the locomotive was restricted to King's Cross-Leeds-King's Cross workings only. The locomotive remained on the restricted list until 19th August, when it was accepted for classified repair

Conclusions: 11.09.75 DME KX states: Although the locomotive could not be faulted at the time there were further reports of this engine cutting out & when the locomotive was accompanied in traffic by my Technical Inspector on 12.08.75 he found that the cause was insufficient wipe on the AFC1 interlocks. I am inclined to think that this was the cause of the casualty, although this is a most unusual defect

Conclusions: 12.02.76 DME KX states: I am puzzled as to say why a mechanical casualty has been initiated for this incident, particularly as the locomotive worked to King's Cross to complete its diagrammed working. I suggest therefore that the casualty should either be cancelled or if seven minutes delay was incurred before the AWS was isolated then this should be taken up with the driver & the casualty coded as Non-mechanical
.

Nevertheless I accept that my control should in the circumstances have specially checked that the locomotive had sufficient fuel to reach Leeds, and I have taken up with the staff concerned accordingly.

This wiring & connections have always been a potential source of trouble & in view of these two recent casualties I am arranging for a campaign check on all locomotives allocated to this division (DME KX)

Conclusions: Please see my letter dated 9th November 1972 regarding grease found in the overspeed governor of locomotive 9001, and its possible effect on the resetting of the pawl. following this latter incident it is understood that Doncaster Works have been greasing these governors by mistake, and a campaign check is now in progress on Finsbury Park allocated locomotives. During the course of this it was found that No.1 engine overspeed governor of locomotive 9007 was heavily greased. I therefore await 'Red Label' inspection report on the Martinair unit with interest, as if no fault is found I would suggest that greasing of the overspeed governor was the cause of this casualty (DME KX).

20.03.74 - Motive power Inspector (Doncaster) states: Driver EH Coultish is a driver transferred to Doncaster from Goole & until recently learning the route to King's Cross had no experience over this section of line, therefore would not be familiar with the watering points located at Peterborough, there is no reference to these points in the Sectional Appendix or SOI although they do exist on the south-east of platform No.2 and north-east of No.3 platform
. According to the Southern Area Sectional Appendix (page 332) the hydrant covers are painted green so that they may be readily distinguished. The covers at Peterborough are not painted and are difficult to distinguish. In my opinion the instructions should be amended in the Southern Area Appendix to include Peterborough and the covers painted green. This would draw attention to drivers that hydrant points still exist.

Following the return of this locomotive to the King's Cross division the No.1 header tank was checked for a hole in the topping up stand pipe but could not be faulted. It was ridden with in traffic on 11th June after further reports of high water temperature on No.1 engine on both 6th & 10th June. It was then found that the engine experienced a high water temperature shut down at 190°F with 18°F differential across the radiators. The locomotive was therefore accepted on 17th June for main works attention to No.1 radiators

The loco was whistled off prior to York in order to give control enough time for a loco change. Driver Handles did not want to hand over a defective loco to the relieving set at Newcastle without an attempt to replace it. It was not his decision for the loco to come off at York, he acted on the instructions received
. Driver Handles was reminded that whistling off indicated loco failure and control no doubt acted upon this.

At Leeds City 200 gallons boiler water registered on gauge. The driver I relieved on the up train at Leeds City said he noticed water leaking from the tank as it neared top when being filled. By the time we passed through Peterborough the water gauge registered less than 100 gallons. Soon after passing Abbots Ripton a fault light indicated low fuel. I had the boiler shut down and as I passed St. Neots & Sandy signal boxes I blew the horn and signalled that I wanted a fresh engine. When I examined the engine on stopping at Hitchin water was leaking from holes underneath. One fuel tank was on zero, the other showed 100 gallons. At no time on the road did I notice any bump or anything to place where this damage could have happened."

Driver J. Crate (King's Cross) reports "The first fault occurred when I closed the controller to reduce speed for the 30 mph speed restriction at New Barnet on the down main line. A bright blue light appeared on the drivers desk. This was No.2 engine showing high water temperature. I could not immediately regain power when I re-opened the controller and brought the train to a stop at New Barnet No.1 home signal. After opening and closing the controller twice I regained power also blue light dimmed. The second fault occurred on the approach to Hatfield, a bright blue light reappeared on the desk due to No.2 engine showing high water temperature again. When closing the controller No.1 engine cut out and stopped for no apparent reason. They advised stopping at Hitchin for a check by fitters. I was able to re-start No.1 engine which gave no further trouble and I opened all engine compartment windows to try to assist cooling of No.2 engine. After a check by fitters at Hitchin the locomotive gave no further trouble apart from No.2 engine continuing to show a bright blue light when working hard. The load of this train was 13 Cartics total tonnage 699."

He reported some flames by engine room step, this was dealt with by carbon dioxide bottle and I kept train moving slowly to clear main to slow road at Newark north to enable trains behind mine to have a path. When I stopped at I investigated and saw that smouldering was taking place in an inaccessible place, so fire brigade had to deal with it.

This second casualty would have been avoided if the driver of the first casualty had made an entry in the drivers repair book as the locomotive would not then have been allowed into service without being accompanied by technical staff. With regards to the smouldering of the exhaust drum tank lagging, I would point out that this locomotive had only been released from Doncaster Works on 18.09.70 after attention to flashover damage, though there is nothing in the repair report to indicate that the lagging was touched.

I would therefore suggest a rewording of the examination schedule to effect that if there is insufficient coolant left in the overflow tank to top up each header tank with the emergency hand pump then a special examination should be carried out for coolant leaks (this would of course include inspection of all hoses).
