The Chronicles of Napier has been a labour of love for the past twenty-five years, however, hosting a website is not free
and although the yearly cost may only be approximately £100 it has been thus for its duration (don't tell my wife!).

Traction Casualty Reports 07.01.77 - 14.12.79. |
1977 |
|
07.01.77 |
King's Cross Loco.
'A' exam.
1S66 20:15 King's Cross - Edinburgh, to
Doncaster (4 minute delay Doncaster - 47522
forward).
Casualty report states: Boiler defective -
unable to fault apart from noting that fuel
solenoid valves were not of the approved type. |
08.01.77 |
Finsbury Park TMD.
Repairs - fuel solenoid valves changed. Type
ACD2 removed & replaced by type AC0/P52/2 &
boiler fully tested. |
13.01.77 |
Finsbury Park TMD.
Boiler test.
1S66 20:15 Kings Cross -
Edinburgh, to Newcastle (21 minute delay at Darlington - 46042 forward).
Casualty report states: boiler defective - water
pump not lifting water. |
14.01.77 |
Finsbury Park TMD.
Repairs - water pump changed. |
04.02.77 |
Finsbury Park TMD.
'B' exam.
1S70 22:15 King’s Cross - Aberdeen, "The Night
Aberdonian" to Doncaster (11 minute delay Doncaster - 47047
forward).
Casualty report states: boiler failure - water
tank empty.
Conclusions: 28.02.77 DME KX states: The
locomotive had just been released from Finsbury
Park Depot after 'B' examination and boiler
repairs. I can only assume that the water tank
had not been filled to capacity by the depot
and/or a large quantity of water had been used
up whilst investigating the boiler defect -
suitable notice is being taken with staff
responsible. |
05.02.77 |
Doncaster TMD.
Boiler water tank re-filled. |
08.02.77 |
Haymarket TMD.
1E01 07:55 Edinburgh - King's Cross, to
Newcastle (7 minute delay Newcastle - 55003
forward).
Casualty report states: Speedometer defective.
Gateshead TMD.
Repairs - wheel wear compensator adjusted.
1E13 12:40 Edinburgh - Kings Cross, from Newcastle.
1S66 20:15 Kings Cross - Edinburgh, (to Newcastle).
Casualty report for above incident states
further trouble with speedometer reported.
1E39 22:30 Edinburgh - King’s Cross, "The Night
Capitals" (from Newcastle). |
09.02.77 |
Finsbury Park TMD.
Repairs - speedometer gauge changed. |
10.03.77 |
1A16 09:47 Harrogate - King’s Cross, "The Yorkshire Pullman"
(30 minute delay Leeds).
Casualty report states: loss of power with ETH
in operation - loco returned to home depot for
further investigation.
Finsbury Park TMD.
Repairs - unable to fault on above incident but
found No.1 engine would not run due to fuel
transfer pump motor brushes worn out, these were
replaced. |
s12.06.77 |
Gateshead TMD.
1A15 08:36 Newcastle - Kings Cross, to Doncaster (30 minute delay
Doncaster - 55017
forward).
Casualty report states: collision with cattle &
No.2 engine cutting out - No.2 engine low in
coolant.
Doncaster TMD.
Repairs - unable to fault for home depot
attention.
Conclusions: 27.06.77 DME KX states: This
locomotive was subsequently ridden in traffic
between King's Cross & Leeds and back and could
not be faulted. To date there have been no
further reports of No.2 engine cutting out and I
am therefore unable to draw any satisfactory
conclusions. |
s26.06.77 |
1S79 23:35 King's Cross - Edinburgh, "The Night
Scotsman" to Newcastle.
Casualty report states: Boiler CCB continuously
tripping (for home depot attention). |
28.06.77 |
Finsbury Park TMD.
Repairs - boiler TR2 relay
defective, this was changed. |
18.07.77 |
1S70 22:15 King’s Cross - Aberdeen, "The Night
Aberdonian" to Newcastle (47419 forward).
Casualty report states: no boiler - short of
boiler water.
Conclusions: 25.08.77 DME KX states: I was under
the impression that it was the normal practice
for class 55 locomotives on these workings to
require water at Newcastle. I am therefore
surprised that the driver should have failed the
locomotive. |
19.07.77 |
Gateshead TMD.
Repairs - water tanks replenished, boiler run up
and tested, found satisfactory. |
22.07.77 |
King's Cross Loco.
'A' exam.
1S17 10:00 King’s Cross - Edinburgh, "The Flying Scotsman"
(to Darlington/Newcastle) (6 minute delay
Darlington - 47433 forward).
Casualty report states: Speedometer fault.
Conclusions 25.08.77 DME KX states: I can only
assume that the speedometer generator had been
struck in traffic and I am therefore treating
this casualty as 'non-mechanical'.
Gateshead TMD.
Repairs - speedometer generator & cable missing
(this was replaced). |
23.11.77 |
1S70 22:15 King’s Cross - Aberdeen, "The Night
Aberdonian" to Edinburgh (6 minute delay Bathley Lane).
Casualty report states: Temporary loss of power
- power regained by shutting down No.2 engine. |
24.11.77 |
Haymarket TMD.
Repairs - No.2 GFC contact tips defective (these
were renewed). |
28.12.77 |
1A08 08:30 Leeds - King's Cross (7 minute delay Doncaster).
Casualty report states: No.1 engine cut out
(restarted on departure) - fuel transfer pump
brushes worn.
Finsbury Park TMD.
Stopped 14:30 - 'B exam
& repairs (new fuel pump fitted). |
1978 |
|
17.01.78 |
Finsbury Park TMD.
'D' exam. To traffic 21:40.
1N42 23:55 King's Cross - Newcastle (69 minute delay Retford & 4
minute delay Doncaster - assisted to York).
Casualty report states: Total failure,
compressor fault, loss of main air - suspect
compressor governor frozen. Driver reported
fault corrected itself after opening nose end
door to allow in warm air. Locomotive worked
forward from York unassisted. No mentholated
spirits in compressor anti-freeze dispenser.
A note which accompanies this particular
casualty report states: BRB instructions are
that mentholated spirits should not be used in
compressed air systems but local instructions
are in force in this division that it should be
used in severe conditions, as were prevailing on
this particular night. It is interesting to note
therefore that this particular locomotive had
come off Finsbury Park Depot who had, due to an
oversight omitted to fill the anti freeze
dispenser, whilst other class 55 locomotives
which had been serviced at King's Cross had had
their dispensers filled and had worked without
trouble. The handwritten remarks at the foot of
this note merely state 'Balls'. |
20.01.78 |
King's
Cross Loco.
'A' exam.
1S70 22:15 King's Cross - Aberdeen, "The Night Aberdonian"
(delayed 56 minutes KX - 47517 forward).
Casualty report states: boiler failed - fuel
pump seized. |
21.01.78 |
Finsbury Park TMD.
Stopped 11:50 - boiler u/s. Boiler fuel pump
changed. |
09.03.78 |
1A02 06:50 Hull - King's Cross, "The Hull Pullman"
(8 minute delay en-route).
Casualty report states: No.2 engine shut down -
suspect engine room roof Niphon plug open
circuit (Securing ring 'nipped up' further &
locomotive checked in traffic by Technical
Inspector - no faults found). |
15.03.78 |
1S42 16:00 King’s Cross - Edinburgh, "The Talisman"
(8 minute delay King's Cross).
Casualty report states: No.1 end air coupler
pipe leaking at valve cap - temporary repair
carried out by fitting No.2 end pipe at ano.1
end. |
05.04.78 |
Holbeck TMD.
'A' exam.
1A41 21:20 Bradford - King's Cross, from Leeds
to Newark (18 minute delay Newark - 47416
forward 06/04).
Casualty report states: Boiler failure - air
lock at fuel pump. |
06.04.78 |
0B06 Newark - Peterborough.
Peterborough TMD.
Repairs - boiler fuel pump bled. |
27.04.78 |
Finsbury Park TMD.
'C' exam. To traffic 02:00.
1N61 10:45 King's Cross - Newcastle, to Doncaster (13 minute delay
Doncaster - 47172 forward).
Casualty report states: boiler defective - fuel
pump seized.
This is an odd casualty report (concluded
29.06.78) as the TRN stated is 1A01, though the
working is stated as 10:45 KX-Newcastle, it dies
say that the fuel pump was renewed, but not
where . Also it would appear that this move was
in order to get 55007 to works.
Conclusions by DME KX on 28.06.78 are also vague
as they state: I note that this locomotive has
subsequently experienced a fractured fuel tank
(16.05.78), works repairs to same, and fuel
strainer/filter contamination problems, but I am
unable to associate this with the above casualty. |
s02.07.78 |
1A40 21:10 Newcastle - King's Cross, to
Doncaster (32 minute delay Doncaster - 46031
forward).
Casualty report sates: boiler defective - black
smoke from chimney. |
05.12.78 |
1E39 22:30 Edinburgh - King's Cross (38 minute
delay Northallerton).
Casualty report states: brake applied, DSD
defective - driver isolated DSD and loco worked
forward. |
06.12.78 |
King's Cross Loco (no mention in FP register so
assumed at KX).
Repairs - air pipe to DSD reservoir fractured at
union (pipe repaired). |
23.12.78 |
1S31 15:00 King's Cross - Aberdeen, to
Newcastle (10 minute delay Newcastle - 47463 forward).
Casualty report states: Locomotive out of fuel.
Conclusions: 09.03.79 DME KX states: Presumably
a pre-arranged change as locomotive utilised to
work train from King's Cross without refuelling
due to shortage of power.
Gateshead TMD.
Re-fuelled - 807 gallons. |
1979 |
|
02.01.79 |
1A29 16:16 Newcastle - Kings Cross, to York.
Casualty report states: ETH not working.
Conclusions: 26.02.79 DME KX states: I can trace
no drivers booking relevant to this incident,
nor where there any further reports of ETH
trouble. I therefore wonder whether this could
have been due to a defect on the coaching stock,
possibly in view of the snowy weather conditions.
York TMD.
Locomotive tested but unable to fault. |
05.01.79 |
Doncaster TMD.
'A' exam.
5A02 11:00 Grantham - Hornsey, to Peterborough
(24 minute delay Peterborough - 31223 forward).
Casualty report states: No.2 engine passing oil
to exhaust - No.1 engine already pinned out.
Locomotive called to works for No.1 engine
repairs, arrangements made for No.2 engine to be
examined.
Peterborough TMD.
Locomotive taken out of traffic - awaiting works
attention. |
23.02.79 |
1A41 21:20 Bradford - King's Cross (12 minute
delay Doncaster).
Casualty report states: Driver reported brakes
slow applying - brake selection in "vacuum
goods" (brake selector switch placed to "vacuum
passenger" and locomotive worked train forward. |
15.03.79 |
1A14 10:55 Leeds - Kings Cross (28 minute
delay Newark - assisted by 50037).
Casualty report states: Loss of power - No.2
load regulator rate valve seized.
Conclusions: 02.05.79 DME KX states: A rather
unusual failure. The driver could have regained
power by shutting down No.2 engine but states
that by the time he had determined this the
assisting locomotive was available.
Finsbury Park TMD.
Stopped 19:00 - load regulator rate valve u/s. To traffic 20:30. |
16.03.79 |
Haymarket TMD.
1E21 17:00 Edinburgh - King's
Cross, to Newcastle (8 minute delay Newcastle -
47428 forward).
Casualty report states: ETH failure.
Conclusions: 02.05.79 DME KX states: As no
further information is available I am left
wondering whether there may have been a fault on
the coaching stock.
Gateshead TMD.
Locomotive tested - no fault found.
1A40 20:30 Newcastle - King's Cross (190 late
- adverse weather conditions). |
12.07.79 |
Doncaster TMD (PM).
'A' exam (stated on 13.07.79 casualty reports). |
13.07.79 |
1D02 12:05 King's Cross - Hull (17 minute delay
Peterborough).
Casualty report sates: No. 2 engine shut down,
loss of coolant - fitter attended (pumped up
coolant into No.2 header tank & engine
restarted).
Conclusions: 20.08.79 DME KX states: No further
trouble reported against No.2 engine. I must
therefore suspect the servicing attention given
at Doncaster the previous night.
1A28 16:30 Hull - King's Cross, to Doncaster (22 minute delay Doncaster
- 47474 forward).
Casualty report sates: Low in power - regulating
air pipe to No.2 end controller fractured.
Doncaster TMD.
Repairs - Regulating air pipe repaired. |
20.07.79 |
Finsbury Park TMD.
'A' exam (not in FP register).
1D02 12:05 Kings Cross - Hull.
1A28 16:30 Hull - King’s Cross (11 minute delay
at Hull).
Casualty report states: No.1 engine low in
coolant - this was topped up. |
03.08.79 |
Finsbury Park TMD.
1S70 22:15 King's Cross - Aberdeen,
to Carstairs (via Carlisle - 47 minute delay
Lockerbie. 20179 & 20201 assist to Carstairs,
47149 forward 04/08).
Casualty report states: Fire in engine room -
extinguished by crew (No.2 engine exhaust drum
tank split burning coolant & hoses). |

fractured at cone. |
06.11.79 |
1L43 14:05 King's Cross - York
(13 minute delay at Peterborough).
Casualty report states: No.1 engine shut down,
low in coolant due to insufficient coolant in
overflow tank - system topped up when checked at
Peterborough.
York TMD.
Repairs - C6 injector adaptor leaking. |
09.11.79 |
1A31 18:11 York - King's Cross (7 minute delay Peterborough).
Casualty report states: No.1 driver's side cab
door jammed in open position - fitter attended
and freed the door.
Finsbury Park TMD.
Stopped 23:30 - sliding door u/s. |
10.11.79 |
Finsbury Park TMD.
Repairs. To traffic 05:30.
1D00 08:05 King's Cross - Hull, (to
Doncaster) (17 minute delay en-route).
Casualty report states: No.2 engine shut down -
suspect piston defect as connecting rod through
crankcase (locomotive taken out of traffic - for
works attention). No mention of
locomotive removal at Doncaster so may have
worked through and back light engine to
Doncaster. |

air pipe fractured at connection to distributor
mounting block. |
10.12.79 |

|
27.01.75 |
1S32 12:00 King's Cross - Aberdeen, "The Aberdonian"
to Newcastle (27 minute delay Newcastle - 47547
fwd).
Casualty report states: No.2 engine shutting
down and No.1 engine high water temperature -
No.2 engine lubricating oil thermostatic bypass
valve & No.1 engine cooling system and
thermostatic bypass valve both defective.
59,010 miles since last classified repair
(26.09.74).
Gateshead TMD.
Repairs - No.1 engine thermostat set to
emergency & loco returned to home depot. |
29.01.75 |
Finsbury Park TMD.
Repairs - following 55007's casualty on 27/01
further investigation in traffic revealed No.2
engine experiencing low oil pressure due to
lubricating oil thermostatic valve not opening
to radiators. Both valves were changed. |

26.05.75 |
1E40 19:20 Aberdeen - King's Cross,

27/05 - 40145 assist forward).
Casualty report states: loss of auxiliaries -
No.s 13 & 14 battery coils dead, BCC tips
oxidised.
117,288 miles since last classified repair
(26.09.74).
Conclusions: 10.07.75 DME KX states: I note that
the new standard examination schedule shortly to
be introduced for class 55 locomotives calls for
the battery change contactor to be examined at
'B' examinations. This might very well have
prevented this casualty.
Move
replaced:
1A31 16:50 Harrogate - King's

27.05.75 |
Finsbury Park TMD.
Repairs - new battery block fitted & BCC Tips
changed.
1N42 23:55 King's Cross - Newcastle, to
York (4 minute delay York - 47522 forward).
Casualty report states: boiler defective, smoke
& soot being emitted from electrode end of
boiler - boiler burner motor defective (later
changed at Finsbury Park on 'C' exam).
119,000 miles since last classified repair
(26.09.74). |
03.06.75 |
Finsbury Park TMD.
'C' exam & repairs (boiler burner motor changed). |
07.06.75 |

'C' exam - to traffic 19:00hrs. |
s08.06.75 |
Finsbury Park TMD.
1S32 12:00 King's Cross - Aberdeen,
to Newcastle (5 minute delay Newcastle - 55015
forward).
Casualty report states: low coolant.
122,370 miles since last classified repair
(26.09.74).
Conclusions 25.06.75 DME KX states: This
locomotive had been released from Finsbury Park
after a 'C' examination at 19:00 on 7th June and
had stood spare until it worked this train. It
is therefore possible that coolant loss from
hoses had taken place during this period
particularly as these locomotives are not
treated with Boscoprene. I would draw attention
to the fact that this locomotive had a history
of high water temperature troubles until stopped
for radiator change on 13th June.
Gateshead TMD.
Coolant topped up and checked for leaks - no
fault found. |
13.06.75 |
Finsbury Park TMD.
Repairs - radiator change following consistent
high water temperatures. |
10.07.75 |
Finsbury Park TMD.
Casualty report dated 28.07.75 states loco off
'B' exam.
Boiler water tank holed - loco restricted to ETH
and primarily King's Cross-Leeds services only
until shopped for repair. This was not lifted
until 19th August when called for classified
repair - any forays out of the Leeds division
were not sanctioned by depot - see following
move & casualty on 25/07.
1N25 16:30 King's Cross - Newcastle,
"The Tees Tyne Pullman". |
19.07.75 |
1S35 14:00 King's Cross - Aberdeen, to Newcastle
(33 minute delay Newcastle - 47461 forward).
Casualty report states: No.1 engine shut down
when controller opened to leave Newcastle.
Conclusions: 18.09.75 DME KX states: A check
on this locomotives records reveals no further
failure of this type. I therefore wonder if the
engine underspeed switch may have operated &
confused the driver into thinking that the
engine had shut down
Gateshead TMD.
Repairs - unable to fault, loco returned to
traffic. |
25.07.75 |
Haymarket
TMD.
1E43 20:15 Aberdeen - King's Cross, from
Edinburgh to York (9 minute delay York - 47295
forward 26/07).
Casualty report states: boiler defective - CWA
tank holed in several places.
149,170 miles since last classified repair
(26.09.74).
Conclusions: 10.09.75 DME KX states: This would
appear to be misuse of the locomotive by control
as the boiler water tank was found to be holed
during the 'B' examination carried out on 10th
July - consequently the locomotive was
restricted to King's Cross-Leeds-King's Cross
workings only. The locomotive remained on the
restricted list until 19th August, when it was
accepted for classified repair. |
26.07.75 |
York TMD.
Boiler isolated - ETH only. |
04.08.75 |
Finsbury Park TMD.
Casualty report dated 06.08.75 states loco off
'D' exam. |
05.08.75 |
1S16 08:00 King's Cross - Edinburgh, to
Newcastle (15 minute delay en-route & 9 minute
delay Newcastle - 47515 forward).
Casualty report states: No.1 engine shut down,
would not restart.
Conclusions: 11.09.75 DME KX states: Although
the locomotive could not be faulted at the time
there were further reports of this engine
cutting out & when the locomotive was
accompanied in traffic by my Technical Inspector
on 12.08.75 he found that the cause was
insufficient wipe on the AFC1 interlocks. I am
inclined to think that this was the cause of the
casualty, although this is a most unusual defect.
Gateshead TMD.

Conclusions: 12.02.76 DME KX states: I am
puzzled as to say why a mechanical casualty has
been initiated for this incident, particularly
as the locomotive worked to King's Cross to
complete its diagrammed working. I suggest
therefore that the casualty should either be
cancelled or if seven minutes delay was incurred
before the AWS was isolated then this should be
taken up with the driver & the casualty coded as
Non-mechanical.

(04.12.75).
Conclusions: 12.02.76 DME KX states: Ill fitting
cab doors and cab doors which will not stay
closed are a continuous source of trouble on
this class of locomotive, but unfortunately due
to the good will of drivers they do not usually
result in casualties. The problem seems to stem
from a combination of the locks, catches and
draught proofing. I can only again request
special attention to all details at main works
overhaul.

97,250 miles since last classified repair
(04.12.75).
Conclusions: 08.09.76 DME KX states: The obvious
conclusions from this casualty is that the
locomotive had not been properly serviced at
King's Cross prior to working this train & this
is being taken up with the staff responsible.
However in view of this report and the previous
casualty on 15th May the boiler water tanks were
specially checked during examination at Finsbury
Park on 29th May & although the suction strainer
joints were renewed no real defects were found.
It is noted however that when the locomotive was
sent to Doncaster Works for Unclassified Repair
on 03.06.76 two leaking water tanks were found. |

however, it was subsequently checked in traffic
by CM&EE representatives & further investigation
at Finsbury Park on 30.10.76 revealed
considerable quantity of water in ETH jumper
cable connection box giving rise to intermittent
earth faults. It was noted that there was
considerable corrosion inside the junction box
which was bad enough to make it necessary to
renew the terminal post for the train heat
control loop wire connections.

Repairs - examination revealed slight flashover
damage on No.1 main generator, No.1 traction
motor blower fan disintegrated & GFC1 contactor
damaged due to severe overheating caused
apparently by loose or poor connection at
terminal end of cable EA1 - GFC renewed and amp
tags renewed to wires on GFC. New blower fitted,

Casualty report states: Brake application only
in emergency - AV2 diaphragm fitted incorrectly
(upside down).
Conclusions: 22.09.76 DME KX states: I have
checked the history of this locomotive & this
shows the previous attention to the AV" valve on
29.06.76 when it was stripped and cleaned due to
a driver's booking of "slow in dropping vacuum".
However in view of the fact the brake had
apparently functioned correctly since that time
I find it difficult to hold the fitter concerned
at fault. |

1N42 23:55 King's Cross - Newcastle
(14 minute delay Peterborough).
Casualty report states: driver requested fitters
attention due to rough riding and severe
oscillation at 70 mph (no faults found).
Conclusions: 29.09.76 DME KX states: Since this
incident the locomotive has been checked in
traffic by a Technical Inspector who reported it
was somewhat rougher in riding than average and
also specially examined at Finsbury Park but no
apparent cause found. It appears to be worse at
No.2 end and a replacement set of dampers has
therefore been obtained for fitting to No.2
bogie at the next examination, or if there are
any more bookings of rough riding. |

1E11 08:50 Aberdeen - King's Cross,
from Edinburgh to Tweedmouth (47431 forward).
Casualty report states: Loss of power with ETH
on.
Conclusions 29.11.72 DME KX states: I note that
this same locomotive suffered from the same
defect at the end of June (see casualty report
for 27.06.76). It has not however been possible
to ascertain any reason why this particular
locomotive has been in trouble, but this
tendency has also been noted on other locos of
this class. Perhaps this is a matter worthy of
further general investigation. Subsequent
examination at Finsbury Park on 30.10.76
revealed accumulation of water in No.1 end ETH
jumper cable connection box.

No.1 end engine speed valve plastic body
fractured allowing air to escape.
Conclusions: 07.01.77 DME KX states: This
locomotive had been released from Doncaster
Works after Classified Repair only two days
earlier. I would suggest that it is likely that
the No.1 engine speed valve casing may have been
damaged during the cause of this attention. |
29.11.76 |

to Newcastle (8 minute delay Newcastle - 55018

Casualty report states: Both main generators
flashed over - cause of casualty not determined.
Conclusions: 10.01.77 DME KX states: After
release from a Classified Repair on 22.11.76
this locomotive suffered several flashovers. At
the 'B' examination on 8th December the main
generator insulation readings were found to be
low. Subsequently the locomotive had to be
shopped again on 16th December due to engine
aerating and the opportunity was taken to change
both main generators.

specifically called for under the item 1 (g) of
the examination schedule and the last such
inspection was earlier in the day at Holbeck,
after which the locomotive worked Leeds - King's
Cross and was working King's Cross -

course no evidence as to whether the leak was
present at this 'A' examination, but the DME
Leeds may wish to draw this casualty to the

Conclusions: Speedometer failures continue to be
of the more common causes of casualties with
this class of locomotive. I understand that a
modified design of wheel wear compensator
resistor was introduced some time ago and I
await the 'red label' inspection report with
interest to see whether it was one of this type
which failed in this instance (DME Newcastle).

Conclusions: Due to late running of the up
working into King's Cross (12:30 Leeds - KX)
there was insufficient time for refuelling. Also
no advice has been received through the control
organisation that the locomotive had not been
refuelled according to diagram in Leeds.
Nevertheless I accept that my control should in
the circumstances have specially checked that
the locomotive had sufficient fuel to reach
Leeds, and I have taken up with the staff
concerned accordingly.

1S16 08:00 King’s Cross - Edinburgh, to

Casualty report states: loss of power - Control

terminal in the underspeed switch touching the
metal work and one wire insulation chafed
through on the same switch due to incorrect
fitting.
15,000 miles since last classified repair
(06.09.73).
Conclusions: I am advised by the DME Newcastle
that the cable that was off its terminal had an
amp tag fitted which had been made into a spade
terminal for some reason or other. Please see
casualty to locomotive 9018 on 19.09.73 which
was also due to defective wiring in an engine
underspeed switch. This wiring & connections
have always been a potential source of trouble &
in view of these two recent casualties I am
arranging for a campaign check on all
locomotives allocated to this division (DME KX).

'B' exam (Martinair unit changed during exam).
1L34 17:30 King’s Cross - Bradford (18 minute

Casualty report states: No.1 engine shut down
due to overspeed tripped - Martinair unit
defective.
Estimated 16,000 miles since last classified
repair (06.09.73) 180,000 stated!
Conclusions: Please see my letter dated 9th
November 1972 regarding grease found in the
overspeed governor of locomotive 9001, and its
possible effect on the resetting of the pawl.
following this latter incident it is understood
that Doncaster Works have been greasing these
governors by mistake, and a campaign check is
now in progress on Finsbury Park allocated
locomotives. During the course of this it was
found that No.1 engine overspeed governor of
locomotive 9007 was heavily greased. I therefore
await 'Red Label' inspection report on the
Martinair unit with interest, as if no fault is
found I would suggest that greasing of the
overspeed governor was the cause of this
casualty (DME KX).
Finsbury Park TMD.
Repairs - Martinair unit changed. |

1N10 11:00 King's Cross - Newcastle (7 minute

coolant hose between water pump and exhaust
manifold coolant distributor split.
Estimated 16,250 milers since last classified
repair (06.09.73) 180,250 stated!
Conclusions: As this was the first working of
this locomotive after 'B' examination at
Finsbury Park Depot, I think that it is quite
possible that some evidence would have been
present at that time of impending failure. This
casualty is therefore being drawn to the
attention of both the supervisor responsible for
finished work and the fitter concerned (DME KX).

been at least two instances of main generator
armature failures on this class of locomotive
recently. The other one of which I am aware
involved 55002 on 12th December. No doubt the
CM&EE will follow up specially with BREL to

20.03.74 - Motive power Inspector (Doncaster)
states: Driver EH Coultish is a driver
transferred to Doncaster from Goole & until
recently learning the route to King's Cross had
no experience over this section of line,
therefore would not be familiar with the
watering points located at Peterborough, there
is no reference to these points in the Sectional
Appendix or SOI although they do exist on the
south-east of platform No.2 and north-east of
No.3 platform. According to the Southern
Area Sectional Appendix (page 332) the hydrant
covers are painted green so that they may be
readily distinguished. The covers at
Peterborough are not painted and are difficult
to distinguish. In my opinion the
instructions should be amended in the Southern
Area Appendix to include Peterborough and the
covers painted green. This would draw attention
to drivers that hydrant points still exist. |

to the above casualty reports initiated by DME
Newcastle & DME Doncaster respectively, I would
point out that both incidents involved the same

No.1 engine header tank emptying through
three-way header tank cock when cock in closed
position.
98, 000 miles since last classified repair
(06.09.73).
Conclusions: 21/06 DME KX states: see also
casualty for previous day. Following the return
of this locomotive to the King's Cross division
the No.1 header tank was checked for a hole in
the topping up stand pipe but could not be
faulted. It was ridden with in traffic on 11th
June after further reports of high water
temperature on No.1 engine on both 6th & 10th
June. It was then found that the engine
experienced a high water temperature shut down
at 190°F with 18°F differential across the
radiators. The locomotive was therefore accepted
on 17th June for main works attention to No.1
radiators.

Casualty report states: Boiler defective -
Mowbray control defective.
Estimated 105,000 miles since last classified
repair (06.09.73) 135,000 stated.
Conclusions: 03/07 DME KX states: It would
appear that the Mowbray float chamber had not
been blown down properly to allow the housing &
float unit to become dirty. It is considered
that the float chamber would not have been so
dirty if it had been blown down properly by
staff at Gateshead and no doubt DME Newcastle
will take up suitably with his staff concerned.
09/07 DME Newcastle states: I note the
concluding remarks made by DME KX that Gateshead
staff are responsible for the failure of
locomotive 55007. This was refuted and
suggested that staff at Finsbury Park were at
fault during its last 'B' exam 28.05.74. |

Casualty report states: AWS fault, brakes keep
applying - AWS converter defective.
152,000 miles since last classified repair
(06.09.73).
Conclusions: 26/09 DME KX states: I consider
that in the circumstances the driver should have
isolated the AWS and proceed forward without
causing delay or casualty. I am therefore
treating this casualty as 'non mechanical' and
perhaps the DME Doncaster may take up with the
driver concerned.

King's Cross Loco.
1S17 10:00 King's Cross - Edinburgh, "The Flying Scotsman"
to York (21 minute delay York - 47407 forward).
Casualty report states: No.1 cab DSD pedal
defective - switch defective.
Driver Handles (Gateshead) was interviewed to
establish reason for delay. 55007 is fitted with
experimental DSD equipment and had to be
isolated because of intermittent trouble before
passing Doncaster. The loco was whistled off
prior to York in order to give control enough
time for a loco change. Driver Handles did not
want to hand over a defective loco to the
relieving set at Newcastle without an attempt to
replace it. It was not his decision for the loco
to come off at York, he acted on the
instructions received. Driver Handles was
reminded that whistling off indicated loco
failure and control no doubt acted upon this.
Conclusions: 26/09 DME KX states: I consider
that in these circumstances the driver should
have isolated the DSD and proceeded forward, so
as to avoid delay and a casualty.

(loco taken out of traffic, for works attention).

28.12.74 |
1S42 16:00 King's Cross - Edinburgh, "The Talisman"
to Darlington (13 minute delay Darlington -
47421 forward).
Casualty report states: No.2 engine lubricating
oil level low.
55,000 miles since last classified repair
(26.09.74).
Conclusions 16.01.75 DME KX states: I note the
details given by DME Newcastle but I do not
consider that 15 gallons of lubricating oil is
significant and the cause of this casualty. The
locomotive was again in trouble three days later
with No.2 engine shutting down when the LPA plug
in the circuit to No.2 engine run solenoid was
found to be loose. After giving this attention
there have been no further reports of engine
shut-down and I am inclined therefore to think
this was the true cause of this casualty.

15 gallons lubricating oil supplied, loco
returned to traffic. |

08.01.70 |
1S32 12:00 King's Cross - Aberdeen,
to Edinburgh (delayed 6 minutes at Darlington & 18 minutes at
Newcastle). Casualty report states: Short of water - a misunderstanding at
Darlington when the guard requested a fresh engine at Newcastle. Driver Blacklock (Gateshead) states that he did not request a loco
change but intended to take water at Newcastle, which he did.
Boiler water tank was filled at Newcastle and loco continued on this
working (water feed slower than normal due to the effects of the
prevailing weather). 74,000 miles since last classified repair (19.08.69). |
13.01.70 |
King's Cross Loco. 1L16 13:20 King's Cross - Bradford. 1A33 17:20 Bradford - King's Cross (delayed 25 minutes at Hitchin
with low fuel - assisted forward by 5905). Casualty report states: Driver G. Elliott reports "On 13.01.70 I
worked 1L16 down & 1A33 up with 9007, both fuel & water tanks full
when leaving King's Cross.
At Leeds City 200 gallons boiler water registered on gauge. The
driver I relieved on the up train at Leeds City said he noticed
water leaking from the tank as it
neared top when being filled. By the time we passed through
Peterborough the water gauge registered less than 100 gallons. Soon
after passing Abbots Ripton a
fault light indicated low fuel. I had the boiler shut down and as I
passed St. Neots & Sandy signal boxes I blew the horn and signalled
that I wanted a fresh engine.
When I examined the engine on stopping at Hitchin water was leaking
from holes underneath. One fuel tank was on zero, the other showed
100 gallons.
At no time on the road did I notice any bump or anything to place
where this damage could have happened." Both right hand fuel tanks and boiler water tank holed (loco to be
taken out of traffic for works attention). |

24.01.70 |
1S60 19:40 King's Cross - Aberdeen
"The Aberdonian",
to Peterborough (delayed 28 minutes between St. Neots & Peterborough
- 1763 forward). Casualty report states: Loss of lubricating oil on No.2 engine -
elbow at end of lubricating oil outlet manifold from BC crankshaft
leaking
badly due to failure of the Araldite joint between brass elbow and
aluminium flange (NB this elbow is non-standard as it has been
modified
for the fitting of ETH apparatus). the Araldite joint was remade
(previous casualty on 28.12.69 states this joint remade at
Gateshead). 76,000 miles since last classified repair (19.08.69). |
17.02.70 |
1N27 18:00 King's Cross - Newcastle
(failed King's Cross, train delayed 19 minutes - 9012 forward). Casualty report states: Loco lights failed due to blown lighting
fuse - wiring burnt on charging & lighting socket on boiler. Water in battery boxes & battery cell cracked (New battery fitted
and wiring renewed). 86,000 miles since last classified repair (19.08.69). |
18.02.70 |
Finsbury Park TMD. Casualty report dated 20.03.70 states loco off repair. |
19.02.70 |
(1Lxx/1Axx - would account for 500 miles between casualties if
mileage correct). 1N27 18:00 King's Cross - Newcastle (failed King's Cross,
train delayed 20 minutes - 1104 forward). Casualty report states: Lighting fuses blown - wiring from charging
socket burnt inside conduit (new charging socket fitted). 86,500 miles since last classified repair (19.08.69). Follow up
report dated 20.03.70 states: This locomotive had just been released
from Finsbury Park depot following repairs
from a previous casualty due to lighting fuses blowing. It would
appear that insufficient attention was given after the earlier
casualty as further wiring in the conduits to the charging and
lighting sockets was found to be burnt out after this second
casualty. Suitable notice has been taken with staff concerned. |

Haymarket TMD. 1E42 19:15 Aberdeen - King's Cross,
"The Aberdonian"
from Edinburgh (delayed 120 minutes at St. Neots (08/03) - 1990

governor frozen (compressor started up whilst being assisted at
King's Cross). Driver J. Bush reports "When working 1E42 Doncaster to King's Cross
engine No.9007. The brake went on at St. Neots. I stopped at signal
35
with loss of main air (compressor stopped) time 08:10. When being

20.03.70 |
Finsbury Park TMD. 4S42 21:05 Dagenham - Bathgate, (from Finsbury Park to Doncaster)
(delayed 3 minutes at New Barnet, 7 at Hatfield & 10 at Hitchin). Casualty report states: No.1 engine cutting out - header tank low in
coolant (header tank topped up). Driver J. Crate (King's Cross) reports "The first fault occurred
when I closed the controller to reduce speed for the 30 mph speed
restriction at
New Barnet on the down main line. A bright blue light appeared on
the drivers desk. This was No.2 engine showing high water
temperature.
I could not immediately regain power when I re-opened the controller
and brought the train to a stop at New Barnet No.1 home signal.
After opening and closing the controller twice I regained power also
blue light dimmed. The second fault occurred on the approach to
Hatfield,
a bright blue light reappeared on the desk due to No.2 engine
showing high water temperature again. When closing the controller
No.1 engine
cut out and stopped for no apparent reason. They advised stopping at
Hitchin for a check by fitters. I was able to re-start No.1 engine
which
gave no further trouble and I opened all engine compartment windows
to try to assist cooling of No.2 engine. After a check by fitters at
Hitchin
the locomotive gave no further trouble apart from No.2 engine
continuing to show a bright blue light when working hard.
The load of this train was 13 Cartics total tonnage 699." 93,000 miles since last classified repair (19.08.69). Follow up report dated 24.03.70 states: This locomotive had just
been released from Finsbury Park after repairs, where due to an
omission by
supervisory staff the cooling system had not been topped up before
return to traffic. The supervisor has been spoken to and 'put right'
for the future. |

Haymarket TMD. 'A' exam. 1E01 08:00 Edinburgh - King's Cross, to Newark
(loco fire - delayed 67 minutes 1102 forward).
PIC Casualty report states: Loco on fire - loco to works for classified
repair. Driver F. Hudson (King's Cross) reports "Fire caused by electric
flashover between Carlton and Cromwell signal boxes, smoke entered
cab approaching Newark Crossing distant, so I immediately braked
train to about 15mph and asked second man to see the reason.
He reported some flames by engine room step, this was dealt with by
carbon dioxide bottle and I kept train moving slowly to clear main
to slow road at Newark north to enable trains behind mine to have a
path. When I stopped at I investigated and saw that smouldering
was taking place in an inaccessible place, so fire brigade had to
deal with it. 102,140 miles since last classified repair (19.08.69). |

1E11 12:00 Edinburgh - King's Cross,

forward). Casualty report states: Both engines cut out - servicing
mismanagement (short of fuel). Low fuel light not working - locomotive returned to Finsbury Park
where it was found that the fault light relay was defective and
had to be renewed. Recommendations: Even if the fault light relay had not been
defective a casualty would not have been avoided. This should
therefore be coded as non-mechanical and no doubt the depot engineer
at Haymarket will investigate the apparent lack of servicing. 26,201 miles since last classified repair (24.04.70). Gateshead TMD. Re-fuel: 750 gallons. |

this switch had been nearly three months earlier in

is not considered that anyone can be held at fault. |

1L22 15:55 King's Cross - Harrogate, to Leeds
(31 minute delay at Essendine & 50 minutes at Monkswood - 1829 &

Flashover main generator and subsequent loss of power. Conclusions: Although this is an isolated instance on this
particular locomotive, the number of flashovers which have occurred
in recent weeks on this class of locomotive is a cause for concern.
It is hoped that the modifications and attention in works now proposed will alleviate the position. Loco hauled out of
division to Doncaster works for attention. 75,000 miles since last classified repair (24.04.70). |

1N27 17:30 King's Cross - Newcastle,
to Peterborough (12 minute delay at Hitchin & 14 minutes at
Peterborough - 1761 forward). Casualty report states: fumes in engine room and smoke from lagging
around No.2 engine exhaust drum (drum tank lagging
removed at Finsbury Park & tested no gas leak found). Driver
A. Martin (King's Cross) reports "No.2 engine shut down.
Smoke emitting from lagging around exhaust drum. Further examination
showed nothing smouldering after No.2 engine had
been shut down for some time." Conclusions: see casualty for this
locomotive two days earlier. This second casualty would
have been avoided if the driver of the first casualty had made an
entry in the drivers repair book as the locomotive would not
then have been allowed into service without being accompanied by
technical staff. With regards to the smouldering of the
exhaust drum tank lagging, I would point out that this locomotive
had only been released from Doncaster Works on 18.09.70
after attention to flashover damage, though there is nothing in the
repair report to indicate that the lagging was touched. 77,410 miles since last classified repair (24.04.70) - 67410

reading 6mph slow at 90mph - wheel wear compensator
out of adjustment (wheel wear compensator adjusted - speedometers
tested and found satisfactory). 122,000 miles since last classified repair (24.04.70). 31.12.70 Conclusions: Wheel wear compensator was deliberately
adjusted to 49" as there was no speedometer available at
York and it was found by staff that the speedometer could be made to
register correctly by altering the compensator, advising
further attention at home depot. This was done and it was found
necessary to adjust the compensator back to 43".
The speedometer could not be faulted and there were no further
reports to the locomotive. York TMD. Repairs. |

Holbeck TMD. 'A' exam. 1A04 07:15 Bradford - King's Cross, to Doncaster (24 minute delay
Doncaster - 1580 forward). Casualty report states: Air leak on brake system - brake valve
having to be moved to overcharged position to release brakes (no
fault found). 1,230 miles since last classified repair (05.02.70). Conclusions: As this locomotive had only just been released from a
main works classified repair it is understood that it was examined
by both
Doncaster Works (Mr. I. Wells) and CM&EE headquarter's staff but
could not be faulted. It was subsequently accompanied in traffic by
the
CM&EE Chief Inspector, Mr. Edwards and again could not be faulted. |

No.2 nose end - work hardening of pipe under vibration (pipe
removed,
repaired and refitted). 10,250 miles since last classified repair (05.02.71). Conclusion: The pipe concerned is one of those which have been
present since the locomotive was built and is, I understand, of
relatively
thick gauge copper. It is however most unusual for this particular
pipe to fail, so I do not consider any special action necessary. Gateshead TMD. Repairs. |

when ETH switch on normal power (CM&EE
staff attended - loco sequence tested unable to fault). 9,300 miles since last classified repair (21.12.71). Conclusion: I understand that before departure 9000 tried twice to
obtain ETH, on third occasion OK. I can only
assume that this defect was due to an earth fault on the coaching
stock and consider that this
casualty should be cancelled. |

lubricating oil - jubilee clips too slack allowing main lub oil
flexible pipe to radiator to disconnect from the radiator (hose
replaced & clips tightened). 15,120 miles since last classified repair (21.12.71). Conclusion: This has been a not uncommon defect with Deltic
locomotives working in very cold weather. I believe that
sometime ago the CM&EE took up with BREL regarding the security and
correct clipping of the hoses and this is the first
instance of failure of which I am aware for sometime. |

unrepairable - window frame sealing rubber blown out. 35,910 miles since last classified repair (18.12.71). Conclusion: This is a defect which does not occur time to time on
these locomotives, although as far as I am aware this is the
first time a casualty has been incurred. Also I would point out that
this locomotive has a particularly bad record over several years of ill-fitting doors and windows and distorted cabs, with a
recent incident of a fracture several inches long between the
windscreens and the cab side & special attention to the cabs was
requested on the shopping proposal for the last classified repair. York TMD. Repairs - window frame made secure for attention at home depot. |

temperature - radiators defective. 65,690 miles since last classified repair (18.12.71). Follow up report dated 31.05.72: Immediately following this casualty
the thermostatic by-pass valve was suspected and changed
and the radiator blocks cleared externally at depot. This however
did not prove effective and following a repeat of this trouble
the locomotive was sent to Doncaster Works for radiator change,
since which it has performed satisfactorily. In view of the
troubles with radiators on this locomotive and also 9001 a few weeks
earlier, perhaps the CM&EE will follow up with BREL regarding the general question of radiator attention in works and
defects found on locomotives 9001 & 9007 in particular. |

GFC2 contactor tips corroded (tips renewed). 75,560 miles since last classified repair (18.12.71). Conclusions: Although this locomotive has run a considerable period
since examination it is felt the quality of
workmanship at the last examination leaves something to be desired
and the matter is being taken up with the
man concerned. |

King's Cross Loco. 'A' exam. 1N42 23:55 King's Cross -Newcastle (10 minute delay King's Cross). Casualty report sates: DSD defective, No.2 end vigilance warning
horn not sounding (Vigilance warning horn armature out of
adjustment (DSD isolated & loco worked through to Newcastle). Driver E Haddon (King's Cross) reports: "The DSD buzzer was not
working, so not getting the buzzer to cancel the brakes.
We requested fitters and they isolated the deadman." 85,690 since last classified repair (18.12.71). Conclusions (11/08): This is the second case of which I am aware of
one of these horns requiring adjustment, the previous one
involved 9020, I consider this defect should not have caused
casualty and I have taken it up with driver as to why a ten minute delay was incurred in isolating the equipment (DME King's Cross). |

loss of coolant - No.1 engine hose No.18 leaking (jubilee clips
tightened & system checked when engine run). 91,820 miles since last classified repair (18.12.71). Conclusions (22/08): The hose which was leaking was in the radiator
roof compartment and one which in practice is not normally
noticed during the course of 'A' examination. I would therefore
suggest a rewording of the examination schedule to effect that if
there
is insufficient coolant left in the overflow tank to top up each
header tank with the emergency hand pump then a special examination
should be carried out for coolant leaks (this would of course
include inspection of all hoses). (DME Newcastle). Gateshead TMD. Repairs. Move replaced:

stopped and would not restart (No.2 fan shaft bolts sheared causing
fan shaft to come adrift, strike and break the overspeed trip
assembly and damage fire detector holder. 20,000 miles since last classified repair (21.09.72).
Conclusions: I would draw attention to the fact that there have
been two other cases recently of similar failures (9014 on 30.09.72
& 9017 on 13.11.72) (DME Leeds). York TMD. Repairs - fire detector holder renewed and CCB reset without
